細(xì)胞色素P450基因多態(tài)性對(duì)圍絕經(jīng)期婦女潮熱易感性的影響

【摘要】:目的研究性激素水平及雌激素代謝酶相關(guān)基因細(xì)胞至色素P450(cytochrome P450,CYP)多態(tài)性對(duì)圍絕經(jīng)期婦女潮熱易感性的影響。方法選取2006年10月至2008年5月西安交通大學(xué)第一附屬醫(yī)院體檢中心進(jìn)行健康體檢的328名圍絕經(jīng)期婦女進(jìn)行病例對(duì)照研究,將其分為潮熱組(n=156)和無潮熱組(n=172)。采用放免法測(cè)定血清卵泡刺激素(follicle stimulating hormone,FSH)、黃體生成素(luteinizing hormone,LH)、雌二醇(estradiol,E2)和睪酮(testosterone,T),酶免法測(cè)定性激素結(jié)合球蛋白(sex hormone binding globulin,SHBG)和硫酸脫氫表雄酮(dehydroepiandrosterone sulfate,DHEA-S)。應(yīng)用聚合酶鏈反應(yīng)-限制性片段長度多態(tài)性方法分析兩組婦女CYP17 Msp A1Ⅰ、CYP19Rsa I、CYP1A1 Msp I和CYP1B1 Eco57I基因多態(tài)性。結(jié)果與無潮熱組婦女比較,潮熱組婦女血清FSH水平增高(P0.01),而E2和SHBG水平降低(P0.05)。CYP1B1 Eco57I基因多態(tài)性在潮熱組和無潮熱組婦女中的分布差異有統(tǒng)計(jì)學(xué)意義(P0.05);攜帶CYP1B1 Eco57I基因多態(tài)性的婦女潮熱發(fā)生的風(fēng)險(xiǎn)為未攜帶婦女的1.25倍(OR=1.25,95%CI:1.03~1.82);中重度潮熱發(fā)生及潮熱持續(xù)時(shí)間1年的風(fēng)險(xiǎn)分別為未攜帶婦女的1.28倍和1.35倍。結(jié)論血清E2水平降低可能是潮熱發(fā)生的原因之一。CYP1B1 Eco57I基因多態(tài)性增加?jì)D女潮熱發(fā)生、潮熱程度和持續(xù)時(shí)間的風(fēng)險(xiǎn)。
