【摘要】：目的探討合并2型糖尿病(T2DM)的高血壓患者與單純?cè)l(fā)性高血壓患者動(dòng)態(tài)血壓水平及血壓變異性的相關(guān)性。方法以800例合并T2DM的原發(fā)性高血壓患者及800例單純?cè)l(fā)性高血壓患者為研究對(duì)象,監(jiān)測(cè)研究對(duì)象日間、夜間及24 h收縮壓及舒張壓變異性,并進(jìn)行對(duì)比分析,研究原發(fā)性高血壓患者血壓變異性與糖尿病的相關(guān)性。結(jié)果高血壓合并T2DM組24 h平均收縮壓、夜間平均收縮壓均明顯高于高血壓組(均P0.05),高血壓合并T2DM組收縮壓晝夜差值、舒張壓晝夜差值均明顯低于高血壓組(均P0.05);高血壓合并T2DM組24 h收縮壓標(biāo)準(zhǔn)差、日間收縮壓標(biāo)準(zhǔn)差、日間收縮壓變異系數(shù)、夜間收縮壓標(biāo)準(zhǔn)差、夜間收縮壓變異系數(shù)、夜間舒張標(biāo)準(zhǔn)差、血壓晨峰均明顯高于高血壓組(均P0.05);經(jīng)相關(guān)分析發(fā)現(xiàn),患者血糖水平與24 h收縮壓標(biāo)準(zhǔn)差、24 h收縮壓變異系數(shù)、日間收縮壓標(biāo)準(zhǔn)差、日間收縮壓變異系數(shù)、日間舒張標(biāo)準(zhǔn)差、日間舒張壓變異系數(shù)、夜間舒張標(biāo)準(zhǔn)差、血壓晨峰呈正相關(guān)(均P0.05)。結(jié)論原發(fā)性高血壓合并T2DM患者血壓變異性較單純?cè)l(fā)性高血壓患者增大,提示糖尿病合并原發(fā)性高血壓時(shí)對(duì)患者晝夜血壓調(diào)節(jié)損害較大并可能與血糖水平有關(guān)。
[Abstract]:Objective to study the patients with type 2 diabetes mellitus (T2DM) patients with hypertension and simple correlation in patients with essential hypertension ambulatory blood pressure levels and blood pressure variability. Methods 800 patients with T2DM in patients with essential hypertension and 800 cases of primary hypertension patients as the research object, the monitoring object of study during the day, night and 24 h systolic blood pressure and diastolic blood pressure variability, and the analysis of correlation between patients with essential hypertension blood pressure variability and diabetes. Results of hypertension combined with T2DM group 24 h mean systolic blood pressure, nighttime average systolic blood pressure was significantly higher than that of the high blood pressure group (P0.05), hypertension group T2DM systolic blood pressure, diastolic blood pressure difference between day and night. The difference between day and night were significantly lower than those in hypertension group (P0.05); hypertension group T2DM 24 h systolic blood pressure standard deviation, daytime systolic blood pressure standard deviation, coefficient of variation of daytime systolic blood pressure, nighttime systolic blood pressure standard deviation, night charge Systolic pressure variation coefficient, night diastolic standard deviation, morning blood pressure peak were significantly higher in hypertension group (P0.05); the correlation analysis showed that blood glucose levels and systolic blood pressure 24 h 24 h standard deviation, coefficient of variation of systolic blood pressure, daytime systolic blood pressure standard deviation, coefficient of variation of daytime systolic blood pressure, diastolic daytime daytime standard deviation. The coefficient of variation of diastolic blood pressure, nighttime diastolic standard deviation, correlation of morning blood pressure peak positive (P0.05). Conclusion hypertension blood pressure variability in patients with T2DM compared with patients with hypertension increased, suggesting that diabetic patients with primary high blood pressure in patients with circadian blood pressure regulation and greater damage may be related to blood glucose levels.
