【摘要】：目的:觀察益腎利濕化瘀法結(jié)合B超引導(dǎo)下穿刺抽液硬化術(shù)治療單純性腎囊腫的遠期療效是否優(yōu)于單純穿刺抽液硬化術(shù)。方法:使用隨機、對照設(shè)計,對直徑4cm的單純性腎囊腫進行干預(yù)治療,對比兩組,即B超引導(dǎo)下穿刺抽液硬化術(shù)(對照組)和加用益腎利濕化瘀法中藥湯劑口服治療28天(治療組),統(tǒng)計3、6、12個月后的療效及復(fù)發(fā)率。結(jié)果:腎囊腫穿刺前:治療組(n=27)、對照組(n=26)囊腫的直徑分別為(6.66土1.90)cm、(6.95±2.10)cm(P0.05),囊腫的體積分別為(129.60±121.50)ml、(144.41±137.94)ml(P0.05);經(jīng)穿刺后,治療組、對照組囊腫的體積分別為(19.54±10.68)ml、(22.82±9.51)ml(P0.05)。3、6、12個月后復(fù)查:治療組囊腫的體積為(6.81 土9.41)ml、(6.81±12.05)ml、(7.08±19.54)ml,明顯小于穿刺后的體積(19.54±10.68)ml(P0.05);對照組 3、6、12 個月后復(fù)查囊腫體積為(41.07±78.92)ml、(42.83±88.40)ml、(46.85±90.17)ml,與穿刺后的體積(22.82±9.51)ml 存在顯著差異(P0.05);經(jīng)治療12個月后治療組囊腫直徑為(1.71±1.50)cm,囊腫體積為(7.08±19.54)ml與對照組囊腫直徑(3.99±2.34)cm,囊腫體積(46.85±90.17)ml相比均存在明顯差異(P0.05);且治療組患者總有效率為96.29%,明顯高于對照組患者總有效率69.23%(P0.05),說明治療組在遠期療效上明顯優(yōu)于對照組。在復(fù)發(fā)率方面,治療組(3.70%)較對照組(37.77%)存在明顯差異(P0.05),表明在中藥干預(yù)治療后單純性腎囊腫遠期復(fù)發(fā)率明顯降低。在癥狀改善方面,治療組改善率92.86%,對照組88.89%,兩組在癥狀改善方面均有很好療效。兩者均無明顯不良反應(yīng)。關(guān)于單純性腎囊腫是否影響腎功能,對53例患者穿刺前和穿刺后比較后發(fā)現(xiàn),肌酐、尿素氮、尿酸、及收縮壓并無明顯差異(P0.05)。結(jié)論:采用益腎利濕化瘀法結(jié)合B超穿刺抽液硬化術(shù)其遠期療效明顯優(yōu)于單純使用穿刺抽液硬化術(shù)治療單純性腎囊腫(直徑4cm者)。
[Abstract]:Objective: to observe whether the long-term effect of invigorating kidney, promoting dampness and removing blood stasis combined with B-ultrasound guided puncture and drainage sclerosis in the treatment of simple renal cysts is better than that of simple puncture and drainage sclerosis. Methods: simple renal cysts with diameter 4cm were treated with random and controlled design.
