【摘要】：目的 探討輸尿管雙J支架管置入術(shù)后再次梗阻的影響因素。方法 回顧性分析121例腫瘤性腎盂積水置入輸尿管雙J管的臨床資料,對腫瘤進(jìn)展、尿鹽結(jié)垢、腎小球?yàn)V過功能、每日尿量、尿路感染、留管時間、下尿路功能及雙J管內(nèi)徑等因素總結(jié),分析輸尿管再次梗阻的原因及預(yù)防方法。結(jié)果 121例患者48例再梗阻,其中腫瘤進(jìn)展、尿鹽結(jié)垢、尿路感染、留置時間是主要影響因素,占87.5%;腎小球?yàn)V過功能、每日尿量及下尿路功能也是重要因素,占12.5%。另外,雙J管內(nèi)徑也是影響再梗阻的因素。通過局部控制腫瘤、改善飲用水及按時換雙J管等相應(yīng)措施,44例未再梗阻,4例因腫瘤進(jìn)展再梗阻改外引流。結(jié)論 輸尿管雙J管置入后,積極抗腫瘤治療、降低飲用水硬度、改善腎功能、多飲水排尿、預(yù)防尿路感染、縮短留管時間、糾正下尿路功能障礙能有效地延緩或預(yù)防雙J管再次梗阻。
[Abstract]:Objective to investigate the influencing factors of re-obstruction after ureter double J stent implantation. Methods the clinical data of 121 cases of malignant hydronephrosis inserted into double J tube of ureter were analyzed retrospectively. the factors such as tumor progress, urinary salt scaling, renal filtration function, daily urine volume, urinary tract infection, retention time, lower urinary tract function and double J tube diameter were summarized. The causes and prevention methods of reobstruction of ureter were analyzed. Results there were 48 cases of re-obstruction in 121 patients, in which tumor progress, urinary salt scaling, urinary tract infection and indwelling time were the main influencing factors, accounting for 8.5%, and renal filtration function, daily urine volume and lower urinary tract function were also important factors (12.5%). In addition, the inner diameter of double J tube is also the factor affecting re-obstruction. By controlling the tumor locally, improving drinking water and changing double J tube on time, 44 cases were not obstructed again and 4 cases were changed to external drainage because of tumor progress. Conclusion active anti-tumor therapy, reduction of hardness of drinking water, improvement of renal function, more drinking water urination, prevention of urinary tract infection, shortening of indwelling time and correction of lower urinary tract dysfunction can effectively delay or prevent re-obstruction of double J tube after double J tube implantation.
