【摘要】：目的觀察腹腔鏡胃底折疊抗反流手術(shù)聯(lián)合胃袖狀切除術(shù)對動物模型的可行性和安全性。方法 2013年12月至2014年5月在柯惠上海臨床培訓(xùn)中心及奧林巴斯上海實驗室對8只小豬行腹腔鏡下保留部分胃底的胃袖狀切除術(shù)聯(lián)合食管裂孔疝修補術(shù)及不同類型胃底折疊術(shù)(包括Nissen、Toupet、Dor胃底折疊術(shù))。記錄術(shù)中情況、手術(shù)步驟、技術(shù)難點及注意事項等。結(jié)果成功完成小豬腹腔鏡胃袖狀切除術(shù)和食管裂孔疝縫合修補術(shù)8次,不同類型胃底折疊術(shù)24次,即Nissen胃底折疊術(shù)、Toupet胃底折疊及Dor胃底前折疊術(shù)各8次。無一例中轉(zhuǎn)開腹,所有實驗動物均在手術(shù)中生命體征保持平穩(wěn),平均手術(shù)時間為(120.2±13.7)min,出血量為5~80(35.8±11.1)m L。術(shù)中1只發(fā)生肝損傷出血,其余均未發(fā)生臟器損傷及吻合口漏。手術(shù)時間隨著訓(xùn)練次數(shù)明顯縮短。結(jié)論腹腔鏡胃底折疊抗反流手術(shù)聯(lián)合胃袖狀切除術(shù)對小豬動物模型安全可行,可為臨床醫(yī)師提供極佳訓(xùn)練機會,有助于縮短學(xué)習(xí)曲線,可能成為治療肥胖合并胃食管反流病或食管裂孔疝的新型抗反流手術(shù)方法。
[Abstract]:Objective to observe the feasibility and safety of laparoscopic gastric fundus folding and anti-reflux surgery combined with gastric sleeve resection in animal model. Methods from December 2013 to May 2014, 8 piglets were treated with laparoscopic sleeve resection combined with esophageal hiatal hernia repair in Ke Hui Shanghai Clinical training Center and Olympus Shanghai Laboratory. Fundus folding (including Nissen,) Toupet,Dor fundus folding). Record intraoperative conditions, surgical steps, technical difficulties and matters needing attention. Results Laparoscopic gastric sleeve resection and esophageal hiatal hernia suture repair were successfully performed in piglets 8 times, 24 different types of gastric fundus folding, namely, Nissen fundus folding, Toupet fundus folding 8 times and Dor anterior fundus folding 8 times. None of the animals were converted to laparotomy. The vital signs of all the experimental animals remained stable during the operation, and the average operation time was (120.2 鹵13.7) min,. The bleeding volume was 5 鹵80 (35.8 鹵11.11) m L.). Liver injury and bleeding occurred in 1 patient, and no organ injury and anastomotic leakage occurred in the rest. The operation time was significantly shortened with the number of training. Conclusion Laparoscopic fundus folding and anti-reflux surgery combined with gastric sleeve resection is safe and feasible in the animal model of piglets. It can provide excellent training opportunities for clinicians and is helpful to shorten the learning curve. It may be a new method of anti-reflux surgery for obesity complicated with gastroesophageal reflux disease or hiatal hernia.
