【摘要】：目的探討微創(chuàng)手術(shù)聯(lián)合中醫(yī)綜合療法治療復(fù)雜性上尿路結(jié)石的臨床療效。方法選取復(fù)雜性上尿路結(jié)石患者240例,隨機(jī)分為實(shí)驗(yàn)組、對(duì)照組,每組各120例。兩組均采用手術(shù)治療,根據(jù)結(jié)石部位及特點(diǎn)采用微創(chuàng)經(jīng)皮腎鏡碎石術(shù)(PCNL)或輸尿管軟鏡鈥激光手術(shù)。實(shí)驗(yàn)組手術(shù)前后加用中藥、針刺、耳穴壓丸的中醫(yī)綜合療法。術(shù)后48~72 h復(fù)查X線(xiàn)片觀(guān)察殘余結(jié)石情況,計(jì)算一期清石率。術(shù)后4周復(fù)查X線(xiàn)片,計(jì)算總清石率。結(jié)果所有手術(shù)均獲成功。實(shí)驗(yàn)組一期清石率為84.7%、術(shù)后4周總清石率為98.9%,對(duì)照組分別為78.1%、92.3%,實(shí)驗(yàn)組一期清石率及總清石率均高于對(duì)照組(P均0.05)。結(jié)論微創(chuàng)手術(shù)聯(lián)合中醫(yī)綜合療法治療復(fù)雜性上尿路結(jié)石的結(jié)石清除率高,中醫(yī)綜合療法能夠促進(jìn)術(shù)后上尿路結(jié)石的排出,有助于彌補(bǔ)微創(chuàng)手術(shù)殘石率高的不足。
[Abstract]:Objective to investigate the clinical efficacy of minimally invasive surgery combined with traditional Chinese medicine (TCM) in the treatment of complicated upper urinary tract calculi. Methods 240 patients with complicated upper urinary tract calculi were randomly divided into experimental group (n = 120) and control group (n = 120). According to the location and characteristics of calculi, minimally invasive percutaneous nephrolithotripsy (PCNL) or ureteral soft lens holmium: YAG laser were used in both groups. Experimental group before and after the operation with traditional Chinese medicine, acupuncture, auricular point pressure pill of traditional Chinese medicine comprehensive therapy. The residual stones were observed by X-ray at 48 h and 72 h after operation, and the rate of primary stone clearance was calculated. X-ray examination was performed 4 weeks after operation to calculate the total stone clearance rate. Results all operations were successful. The rate of primary stone removal was 84.7% in the experimental group, 98.9% in the four weeks after operation, and 78.1% and 92.3% in the control group, respectively. The first stage stone clearing rate and the total stone clearing rate in the experimental group were higher than those in the control group (P 0.05). Conclusion the treatment of complicated upper urinary tract calculi with minimally invasive surgery combined with traditional Chinese medicine (TCM) has a high stone clearance rate. TCM combined therapy can promote the removal of upper urinary tract stones after operation and help to make up for the deficiency of high residual stone rate in minimally invasive surgery.
