【摘要】：半乳糖凝集素-7(Galactose lectin,Galectin-7)是一類能夠選擇性識(shí)別糖結(jié)構(gòu)并與其非共價(jià)結(jié)合的蛋白質(zhì)，它廣泛存在于從低等生物到哺乳動(dòng)物等各類生物體內(nèi)，在組織生長調(diào)節(jié)、細(xì)胞間的黏附、細(xì)胞凋亡、腫瘤發(fā)生及免疫反應(yīng)等多種方面發(fā)揮著重要的生物學(xué)功能。 本課題旨在研究探討galectin-7在人腎移植術(shù)后免疫排斥反應(yīng)中的作用、意義及其與移植免疫排斥反應(yīng)的相互關(guān)系，從而為腎移植排斥反應(yīng)的診斷及治療提供新思路。通過收集2011年12月至2013年10月在解放軍第309醫(yī)院行同種異體腎移植的患者65例，根據(jù)臨床生化指標(biāo)、HE及Masson染色（Banff2005腎移植排斥反應(yīng)活檢診斷標(biāo)準(zhǔn)）將其分為急性排斥組和慢性排斥組，其中急性排斥組34例，男16例，女18例，年齡22-57歲，平均年齡（43±16.3）歲。慢性排斥31例，男17例，女14例，年齡27-54歲，平均年齡（41±17.8）歲。另選取健康供者30例作為健康對(duì)照組，男18例，女12例，，年齡29-37歲，平均年齡（34±13.1）歲。應(yīng)用免疫組織化學(xué)染色法和Western Blot法分別檢測各組腎組織中g(shù)alectin-7的表達(dá)分布情況以及各組血清中g(shù)alectin-7表達(dá)的差異。經(jīng)實(shí)驗(yàn)得出以下結(jié)果，正常組光鏡下腎小球及腎小管結(jié)構(gòu)清晰，未見明顯異常， galectin-7表達(dá)于腎小管管腔處和動(dòng)脈平滑肌細(xì)胞，其他部位未見其表達(dá)。急性排斥組光鏡下腎小球系膜基質(zhì)增生，腎小管明顯上皮炎，腎間質(zhì)內(nèi)可見大量淋巴細(xì)胞浸潤，腎間質(zhì)未見纖維化，腎小管未見萎縮，多數(shù)病例腎小管管腔處可見galectin-7表達(dá)，部分管腔處galectin-7未見表達(dá)。慢性排斥組光鏡下腎小球毛細(xì)血管系膜增生，腎間質(zhì)及基質(zhì)纖維化，腎小管萎縮，未見明顯腎小管上皮炎，動(dòng)脈內(nèi)膜增生，伴少量淋巴細(xì)胞浸潤，部分病例腎小管上皮細(xì)胞可見galectin-7高表達(dá)。Western Blot檢測結(jié)果為：正常對(duì)照組灰度值為（0.32±0.15），急性排斥組灰度值為（0.95±0.11），慢性排斥組灰度值為（1.32±0.19），應(yīng)用SPSS20.0軟件進(jìn)行統(tǒng)計(jì)分量資料以均數(shù)±標(biāo)準(zhǔn)差（）,數(shù)據(jù)采用one-way ANOVA處理，各指標(biāo)間分別作相關(guān)分析，急性排斥組高于正常對(duì)照組，P0.05，差異具有統(tǒng)計(jì)學(xué)意義；慢性排斥組高于正常對(duì)照組，P0.05，差異具有統(tǒng)計(jì)學(xué)意義；慢性排斥組高于急性排斥組，P0.05，差異具有統(tǒng)計(jì)學(xué)意義。本研究得出galectin-7在腎移植術(shù)后排斥反應(yīng)中發(fā)揮作用,可作為移植術(shù)后排斥反應(yīng)監(jiān)測指標(biāo)之一，并可能成為治療移植排斥反應(yīng)的潛在靶點(diǎn)。
[Abstract]:Galactose lectin-7 (Galactose lectin,Galectin-7) is a kind of proteins that can selectively recognize the structure of sugar and bind to it noncovalently. It exists in a wide range of organisms from low-level organisms to mammals and regulates the growth of tissues. Intercellular adhesion, apoptosis, tumorigenesis and immune response play an important role in biology. The purpose of this study is to study the role and significance of galectin-7 in immune rejection after human renal transplantation and its relationship with allograft rejection, so as to provide a new way of thinking for the diagnosis and treatment of renal transplantation rejection. From December 2011 to October 2013, 65 patients underwent renal allograft in the 309th Hospital of the Chinese people's Liberation Army (PLA), according to clinical biochemical indicators, The patients were divided into acute rejection group (n = 34) and chronic rejection group (n = 34) by HE and Masson staining (diagnostic criteria of Banff2005 renal transplantation rejection biopsy). There were 34 patients with acute rejection, 16 males and 18 females, with an average age of (43 鹵16.3) years old (22 鹵57 years old) with a mean age of (43 鹵16.3) years. 31 cases of chronic rejection, 17 males and 14 females, aged 27 to 54 years, mean age (41 鹵17.8) years. In addition, 30 healthy donors (18 males and 12 females, with an average age of (34 鹵13.1) years) were selected as the control group. Immunohistochemical staining and Western Blot method were used to detect the expression and distribution of galectin-7 in renal tissue and the difference of galectin-7 expression in serum of each group. The results showed that the structure of glomeruli and tubules was clear and no obvious abnormality was observed in the normal group. The expression of galectin-7 was found in the lumen of renal tubules and arterial smooth muscle cells, but no expression was found in other parts of the renal tubular lumen and artery smooth muscle cells. In acute rejection group, Mesangial matrix proliferation, renal tubule dermatitis, a large number of lymphocyte infiltration in renal interstitial, no fibrosis in renal interstitial, no atrophy of renal tubule, and the expression of galectin-7 in tubular cavity were observed in most of the cases under light microscope, and there was no fibrosis in renal interstitial tissue and no atrophy of renal tubule in acute rejection group. There was no expression of galectin-7 in some of the lumen. In the chronic rejection group, Mesangial hyperplasia of glomeruli, fibrosis of renal interstitial and matrix, atrophy of renal tubule, no obvious supratubular dermatitis, arterial intimal hyperplasia and a small amount of lymphocyte infiltration were observed in the chronic rejection group. The high expression of galectin-7 in renal tubular epithelial cells was observed in some cases. The gray value of normal control group was (0.32 鹵0.15), that of acute rejection group was (0.95 鹵0.11), and that of acute rejection group was (0.95 鹵0.11). The gray value of chronic rejection group was (1.32 鹵0.19). SPSS20.0 software was used to carry out statistical component data, mean 鹵standard deviation (), data were processed by one-way ANOVA, and correlation analysis was made among the indexes. The acute rejection group was higher than the normal control group, P 0.05, the difference was statistically significant. The chronic rejection group was higher than the normal control group (P 0.05), and the chronic rejection group was higher than the acute rejection group (P 0.05). In this study, we concluded that galectin-7 may play an important role in renal allograft rejection, and may be a potential target for the treatment of renal allograft rejection.
