【摘要】：目的：采用系統(tǒng)評價(jià)方法評估中性粒細(xì)胞明膠酶相關(guān)脂質(zhì)運(yùn)載蛋白（NGAL）在急性腎損傷早期的臨床診斷價(jià)值。 方法：計(jì)算機(jī)電子檢索Pubmed、Cochrane、EMBASE、CNKI、萬方、維普等數(shù)據(jù)庫中與NGAL相關(guān)的文獻(xiàn)，根據(jù)納入排除標(biāo)準(zhǔn)篩選文獻(xiàn)，，提取納入研究特征數(shù)據(jù)。采用QUADAS 2質(zhì)量評價(jià)工具進(jìn)行納入研究的文獻(xiàn)質(zhì)量評價(jià)。利用Meta Disc1.4軟件，根據(jù)異質(zhì)性特點(diǎn)選擇相應(yīng)模型，進(jìn)行診斷統(tǒng)計(jì)量合并。 結(jié)果：初篩到5511篇文獻(xiàn)，嚴(yán)格根據(jù)納入排除標(biāo)準(zhǔn)，最終納入35篇文獻(xiàn)，共納入研究對象8941例，其中AKI組患者2210例。Meta分析結(jié)果顯示尿NGAL與血NGAL的合并敏感度、特異度、綜合受試者工作特征曲線下面積分別為0.73（95%CI:0.71 0.76）、0.73（95%CI:0.72 0.74）、0.8658（SE:0.0201）以及0.80（95%CI:0.77 0.83）、0.76（95%CI:0.74 0.78）、0.8977（SE:0.0214）。 結(jié)論：血、尿NGAL對于患者AKI的發(fā)生預(yù)測，有較好的敏感度和特異度，對患者AKI的早期診斷有重要意義。
[Abstract]:Objective: to evaluate the clinical value of neutrophil gelatinase-associated lipid carrier protein (NGAL) in the early stage of acute renal injury. Methods: the literatures related to NGAL in Pubmed,Cochrane,EMBASE,CNKI, Wanfang and Weip databases were searched by computer, and the data were extracted according to the inclusion exclusion criteria. The literature quality was evaluated by QUADAS 2 quality evaluation tool. By using Meta / Disc1.4 software, the corresponding models were selected according to the characteristics of heterogeneity, and the diagnostic statistics were merged. Results: from the first screening to 5511 articles, strictly according to the exclusion criteria, 35 articles were included in the study. A total of 8941 subjects were included in the study, including 2210 patients in the AKI group. The results of Meta analysis showed the combined sensitivity and specificity of urinary NGAL and blood NGAL. The area under the operating characteristic curve was 0.73 (95%CI:0.71 0.76), 0.73 (95%CI:0.72 0.74), 0.8658 (SE:0.0201) and 0.80 (95%CI:0.77 0.83), respectively. 0.76 (95%CI:0.74 0.78), 0.8977 (SE:0.0214). Conclusion: NGAL in blood and urine has good sensitivity and specificity in predicting the occurrence of AKI in patients. It is important for the early diagnosis of AKI in patients.
