【摘要】：目的：觀察聯(lián)合應(yīng)用針挑療法和口服左卡尼汀治療男性不育弱精子癥的治療效果，探索一種新的臨床治療弱精子癥的中西醫(yī)結(jié)合療方法。 方法：選取2012年6月～2013年12月期間，在我院新醫(yī)針挑科不孕不育門(mén)診就診的弱精子癥患者120例，運(yùn)用數(shù)表法隨機(jī)分為針挑療法組、左卡尼汀組和針?biāo)幝?lián)合組。針挑治療每周一次，左卡尼汀口服液為餐后口服，一次1支（10ml），每日3次，治療周期3個(gè)月。分別記錄患者治療前后精子密度、活率、快速前向運(yùn)動(dòng)精子百分比、前向運(yùn)動(dòng)精子百分比等精液常規(guī)參數(shù)，將結(jié)果進(jìn)行統(tǒng)計(jì)學(xué)分析。 結(jié)果：研究中共脫落病例9例，脫落率7.5%（9/120）。三種不同治療方法各自進(jìn)行組內(nèi)比較顯示，各項(xiàng)精液常規(guī)參較治療前均有顯著的提升，治療前后的差異具有明顯的統(tǒng)計(jì)學(xué)意義（P0.05）。對(duì)治療之后的結(jié)果進(jìn)行組間比較發(fā)現(xiàn)，針挑療法組和左卡尼汀組各項(xiàng)參數(shù)之間的差異沒(méi)有統(tǒng)計(jì)學(xué)意義（P0.05）；針?biāo)幝?lián)合組與其他兩組比較，快速前向運(yùn)動(dòng)精子百分比和前向運(yùn)動(dòng)精子百分比升高更明顯，并且差異都有統(tǒng)計(jì)學(xué)意義（P0.05）；在改善精子密度方面，針?biāo)幗Y(jié)合組較其他兩組升高更多，但與針挑療法組比較，差異沒(méi)有統(tǒng)計(jì)學(xué)意義（t=-1.71，P=0.090.05）；與左卡尼汀組比較，差異顯著（t=-2.26，P=0.030.05）；在改善精子活率方面，治療后三種療法組間相互比較，均無(wú)統(tǒng)計(jì)學(xué)意義（P0.05）。 結(jié)論：?jiǎn)为?dú)應(yīng)用針挑療法或口服左卡尼汀治療，，均能顯著提高快速前向運(yùn)動(dòng)精子百分比、前向運(yùn)動(dòng)精子百分比，有利于提高女方妊娠率，但二者在改善精液質(zhì)量方面無(wú)明顯差異。針挑療法聯(lián)合口服左卡尼汀是一種新的中西醫(yī)結(jié)合治療弱精子癥的方法，其治療效果優(yōu)于單獨(dú)應(yīng)用左卡尼汀或針挑療法，并且該方法操作簡(jiǎn)便，易于掌握，取材方便，安全有效，值得在臨床上應(yīng)用及推廣。
[Abstract]:Objective: to observe the therapeutic effect of combined acupuncture therapy and oral levacarnitine in the treatment of male infertility and asthenospermia, and to explore a new method of integrated traditional Chinese and western medicine for the treatment of asthenospermia. Methods: from June 2012 to December 2013, 120 patients with asthenospermia were selected and randomly divided into three groups: acupuncture group, left carnitine group and combined acupuncture and medicine group. Acupuncture therapy was done once a week, levacanitine oral solution was taken orally after meal, one dose (10ml), 3 times a day, the treatment period was 3 months. Sperm density, motility rate, fast forward motility sperm percentage and forward motility sperm percentage were recorded before and after treatment, and the results were analyzed statistically. Results: 9 cases of shedding cases were studied, the exfoliation rate was 7.5% (9 / 120). The results showed that the semen routine parameters were significantly improved before and after treatment, and the difference before and after treatment was statistically significant (P0.05). After the treatment of the results of comparison between the group found that the acupuncture group and the left carnitine group between the parameters of the difference was not statistically significant (P0.05); Compared with the other two groups, the percentage of fast forward motility sperm and the percentage of forward motile sperm increased significantly in the acupuncture and medicine combination group, and the difference was statistically significant (P0.05). In improving the sperm density, the acupuncture and drug combination group increased more than the other two groups, but there was no significant difference compared with the acupuncture group (t = 1.71, P < 0.090.05). Compared with levocarnitine group, the difference was significant (t _ (2.26) P _ (0.030.05), in improving sperm motility, there was no significant difference between the three treatment groups after treatment (P0.05). Conclusion: acupuncture alone or oral L-carnitine therapy can significantly increase the percentage of fast forward motility sperm and improve the pregnancy rate of women. But there was no significant difference between them in improving semen quality. Acupuncture therapy combined with oral levacarnitine is a new method of treating asthenospermia with combination of traditional Chinese and western medicine, and its therapeutic effect is better than that of single application of left carnitine or acupuncture therapy, and the method is simple, easy to grasp and easy to take materials. Safe and effective, worthy of clinical application and promotion.
