【摘要】：目的：分析我院腎移植術(shù)后并發(fā)肺部感染患者病原菌分布情況及耐藥情況，為臨床抗生素使用提供合理依據(jù)。 方法：回顧性統(tǒng)計(jì)2012年1月-2013年12月年我院收治診斷腎移植術(shù)后并發(fā)肺部感染患者病例，分析其通過痰液或BALF檢出的病原菌菌株分布及藥敏情況，并對(duì)比分析腎移植術(shù)后各階段感染菌株的構(gòu)成。 結(jié)果：收治患者190例次，，取得標(biāo)本481份，檢出169株病原菌株，革蘭陰性菌株93株，革蘭陽(yáng)性菌株60株，真菌16株。主要細(xì)菌株為鮑曼/溶血不動(dòng)桿菌、肺炎克雷伯菌、溶血葡萄球菌、大腸埃希菌等，主要真菌為白色念珠菌。病原菌株中多重耐藥菌株檢出率高，混合感染病例約占51.17%。腎移植術(shù)后不同時(shí)間段肺部感染細(xì)菌菌株構(gòu)成不同。 結(jié)論：（1）我院腎移植術(shù)后肺部感染的病原菌以革蘭陰性菌為主；（2）我院腎移植術(shù)后肺部感染病原菌多重耐藥嚴(yán)重；（3）腎移植術(shù)后肺部感染的細(xì)菌菌株分布構(gòu)成與其移植時(shí)間有關(guān)。
[Abstract]:Objective: to analyze the distribution of pathogenic bacteria and drug resistance in patients with pulmonary infection after renal transplantation in our hospital. Methods: the clinical data of patients with pulmonary infection after renal transplantation from January 2012 to December 2013 were retrospectively analyzed. The distribution and drug sensitivity of pathogenic bacteria detected by sputum or BALF were analyzed. The composition of infected strains in different stages after renal transplantation was analyzed. Results: totally 481 specimens were collected from 190 patients, 169 pathogenic strains, 93 Gram-negative strains, 60 Gram-positive strains and 16 fungi were detected. The main bacteria strains were Baumann / Acinetobacter hemolyticus, Klebsiella pneumoniae, Staphylococcus haemolyticus, Escherichia coli and Candida albicans. The detection rate of multidrug resistant strains in pathogenic strains was high, and mixed infection cases accounted for 51.17%. The composition of bacterial strains of pulmonary infection was different at different time after renal transplantation. Conclusion: (1) Gram-negative bacteria are the main pathogens of pulmonary infection after renal transplantation in our hospital, (2) the pathogens of pulmonary infection after renal transplantation in our hospital are multidrug resistant. (3) the distribution of bacterial strains of pulmonary infection after renal transplantation was related to the time of transplantation.
