Syn、CgA及Ki-67在前列腺癌中的表達(dá)及其臨床意義

【摘要】:目的:探討突觸素(Synaptophysin, Syn)、嗜鉻粒蛋白A(chrormogranin A, CgA)及Ki-67在前列腺癌組織中表達(dá)的臨床意義及Ki-67與前列腺癌的神經(jīng)內(nèi)分泌化(neuroendocrine differentiation, NED)之間的關(guān)系,為前列腺癌在臨床診斷及治療提供有效依據(jù)。 方法:1.選擇前列腺癌(PCa組)標(biāo)本50例,其中高分化癌3例,中分化癌23例,低分化癌24例;因高分化前列腺例數(shù)較少,將高分化和中分化合為H組(Gleason積分≤7),低分化為L(zhǎng)組(Gleason積分7),10例的良性前列腺增生標(biāo)本(BPH組)。本實(shí)驗(yàn)以免疫組織化學(xué)染色法為實(shí)驗(yàn)方法,比較Syn、CgA、Ki-67在前列腺癌及良性前列腺增生中的表達(dá)及其差異,討論Syn、CgA、Ki-67的陽(yáng)性表達(dá)率與前列腺癌的分化、臨床病理分級(jí)之間的關(guān)系。 結(jié)果:1.Syn陽(yáng)性表達(dá)率分別為PCa組56%和BPH組10%,兩組的比較有顯著性差異(P0.05)。CgA陽(yáng)性表達(dá)率PCa組48%和BPH組0%。兩組比較有顯著性差異(P0.05)。Ki-67陽(yáng)性表達(dá)率分別為PCa組74%和BPH組40%。兩組比較有顯著性差異(P0.05)。 2.將前列腺癌組分成H組和L組。Syn、CgA、Ki-67的陽(yáng)性率明顯高于積分低的一組。Syn陽(yáng)性表達(dá)率分別為:H組42.3%;L組70.8%。兩組的比較有顯著性差異(P0.05)。CgA陽(yáng)性表達(dá)率分別為:H組34.6%;L組62.5%。兩組的比較有顯著性差異(P0.05)。Ki-67陽(yáng)性表達(dá)率分別為:H組61.5%;L組87.5%。兩組比較有顯著性差異(P0.05)。 3.采用Syn及CgA同時(shí)陽(yáng)性者為NED陽(yáng)性,其余為NED陰性。H組神經(jīng)內(nèi)分泌化例數(shù)為9例。L組神經(jīng)內(nèi)分化陽(yáng)性例數(shù)為15例。兩組相比較有顯著差異(P0.5)。 4.24例NED陽(yáng)性者中Ki-67陽(yáng)性例數(shù)為22例,其陽(yáng)性率為91.7%,NED陰性者Ki-67陽(yáng)性例數(shù)為13例,其陽(yáng)性率為50%,兩組的比較有顯著性差異(P0.05)。 結(jié)論:1.Syn、CgA、Ki-67。在前列腺癌中陽(yáng)性表達(dá)率明顯高于良性前列腺增生。表明這三種指標(biāo)可用于鑒別良性前列腺增生與前列腺癌。2.syn、CgA、Ki-67的陽(yáng)性率隨著前列腺癌的分化程度升高而降低,即低分化前列腺癌陽(yáng)性率就升高。說明前列腺癌的發(fā)生和發(fā)展均與神經(jīng)內(nèi)分泌化有一定聯(lián)系。3.分化程度越差,神經(jīng)內(nèi)分泌化的前列腺組織越多,這說明神經(jīng)內(nèi)分泌化與前列腺癌的分化和發(fā)展有關(guān)。3.NED陽(yáng)性者Ki-67陽(yáng)性率明顯高于NED陰性者,提示Ki-67與神經(jīng)內(nèi)分泌化有一定關(guān)聯(lián),在判斷前列腺癌預(yù)后方面,有一定指導(dǎo)意義。
