雄性大鼠膀胱出口梗阻致膀胱重構(gòu)的模型比較研究

【摘要】:目的:通過采用恥骨后途徑膀胱頸部分結(jié)扎和會陰途徑球部尿道部分結(jié)扎兩種方法,建立雄性大鼠膀胱出口部分梗阻(Paritial bladder outlet obstraction,pBOO)模型,并對所建模型進(jìn)行鑒定和比較,為pBOO后膀胱重構(gòu)(Bladder remodeling)的深入研究提供一種成活率高,復(fù)制性和穩(wěn)定性較好的動物模型。方法:80只健康雄性Wistar大鼠隨機(jī)分為三組:Ⅰ組為假手術(shù)組(對照組),20只;Ⅱ組為恥骨后途徑膀胱頸部分結(jié)扎組,30只;Ⅲ組為會陰途徑球部尿道部分結(jié)扎組,30只。依據(jù)梗阻時間分別將Ⅰ組、Ⅱ組、Ⅲ組大鼠隨機(jī)分為2周組和4周組,于術(shù)后2周和4周對大鼠行尿動力學(xué)檢測后,完整切取膀胱測其重量,將精囊腺組織和部分膀胱行10%福爾馬林液固定,H-E染色,觀察組織學(xué)變化。結(jié)果:Ⅱ組和Ⅲ組成活率分別為73.3%和80.0%,二者無統(tǒng)計學(xué)意義;Ⅰ組、Ⅱ組、Ⅲ組建模手術(shù)時間分別為(9.75±2.29)、(17.33±3.54)、(10.77±9.44)min,Ⅱ組與Ⅰ組和Ⅲ組比較差異均有統(tǒng)計學(xué)意義(P0.05);Ⅰ組、Ⅱ組、Ⅲ組的2周組和4周組逼尿肌漏尿點壓(Detrusor leak point pressure,DLPP)分別為(26.31±2.32)、(27.34±3.93)、(24.68±2.39)mmHg和(26..42±2.41)、(34.23±3.01)、(32.63±3.20) mmHg,Ⅱ組與Ⅲ組的:2周組比較差異有統(tǒng)計學(xué)意義(P0.05),Ⅰ組的4周組顯著低于Ⅱ組和Ⅲ組的4周組(P0.01),Ⅱ組和Ⅲ組的2周組和4周組比較差異均有統(tǒng)計學(xué)意義(P0.01);Ⅰ組、Ⅱ組、Ⅲ組的2周組和4周組膀胱濕重分別為(0.1486±0.0200)、(0.1881±0.0490)、(0.1797±0.0286)g和(0.1478±0.0236)、(0.2567±0.0476)、(0.2453±0.0378)g,Ⅰ組和Ⅱ組在2周時間點上的膀胱濕重有統(tǒng)計學(xué)差異(P0.05),Ⅰ組和Ⅱ組、Ⅲ組在4周時間點上的膀胱濕重差異有統(tǒng)計學(xué)意義(P0.01),Ⅱ組和Ⅲ組分別在2周和4周不同時間點上的膀胱濕重有統(tǒng)計學(xué)顯著性差異(P0.01)。梗阻2周后膀胱組織結(jié)構(gòu)尚未發(fā)生明顯變化,4周后逼尿肌細(xì)胞增生、肥大,逼尿肌層變厚,平滑肌細(xì)胞之間及肌束之間膠原纖維增生。精囊腺H-E染色可見恥骨后途經(jīng)無明顯擴(kuò)張,而會陰途徑腺腔明顯擴(kuò)張。結(jié)論:恥骨后途徑膀胱頸部分結(jié)扎和會陰途徑球部尿道部分結(jié)扎兩種方法都能成功建立雄性大鼠pBOO模型,與恥骨后途徑相比,會陰途徑成活率高,并發(fā)癥少且容易控制,手術(shù)操作時間短,復(fù)制性和穩(wěn)定性好。
