【摘要】：目的 對(duì)比經(jīng)尿道輸尿管軟鏡下腎囊腫內(nèi)切開(kāi)引流術(shù)和后腹腔鏡下腎囊腫去頂術(shù)臨床療效。 方法 采用回顧性分析,選取40例腎囊腫患者,年齡44歲到75歲,平均年齡61歲,分為兩組,即A組(輸尿管軟鏡下內(nèi)切開(kāi)引流術(shù))和B組(后腹腔鏡下腎囊腫去頂術(shù)),觀察兩組患者治療效果、并發(fā)癥及術(shù)后隨訪(fǎng)結(jié)果。 結(jié)果 兩組手術(shù)均成功,B組因1例患者粘連比較嚴(yán)重而改為開(kāi)放手術(shù),術(shù)中術(shù)后均無(wú)明顯的并發(fā)癥,術(shù)后復(fù)查CT或者B超提示腎囊腫消失或直徑縮小至術(shù)前一半以上。而手術(shù)時(shí)間,術(shù)中出血量,術(shù)后疼痛分?jǐn)?shù)及術(shù)后住院時(shí)間方面,A組均優(yōu)于B組,差異均有顯著意義(P0.05)。隨訪(fǎng)中兩組未發(fā)現(xiàn)腎囊腫復(fù)發(fā)。 結(jié)論 1.輸尿管軟鏡治療腎囊腫是安全、有效的； 2.通過(guò)一定的病例選擇,輸尿管軟鏡內(nèi)切開(kāi)引流術(shù)優(yōu)于后腹腔鏡去頂術(shù)；具有近期微創(chuàng),安全,有效,無(wú)痛苦,并發(fā)癥少,住院時(shí)間短,恢復(fù)快,無(wú)切口的優(yōu)點(diǎn),可作為腹腔鏡治療腎囊腫的補(bǔ)充； 3.輸尿管軟鏡治療外周型腎囊腫存在不足。
[Abstract]:Objective to compare the clinical effects of transurethral soft ureteroscopy with internal incision and drainage and retroperitoneal laparoscopic renal cyst removal. Methods 40 patients with renal cysts, aged from 44 to 75 years, with an average age of 61 years, were divided into two groups: group A (ureteral soft endoscope incision and drainage) and group B (retroperitoneal laparoscopic renal cyst removal). The therapeutic effect, complications and postoperative follow-up results were observed in both groups. Results the operation was successful in both groups. Group B was operated on because of the severe adhesion of one patient. There were no obvious complications during and after operation. CT or B-ultrasound showed that the renal cyst disappeared or the diameter reduced to more than half before operation. The operative time, intraoperative bleeding volume, postoperative pain score and postoperative hospitalization time in group A were better than those in group B (P0.05). No recurrence of renal cyst was found in the two groups. Conclusion 1. Soft ureteroscopy is safe and effective in the treatment of renal cyst. Through certain case selection, soft ureteroscopic incision and drainage was superior to retroperitoneal laparoscopy. It has the advantages of minimally invasive, safe, effective, painless, less complications, short hospital stay, quick recovery and no incision. It can be used as a supplement for laparoscopic treatment of renal cyst. The treatment of peripheral renal cyst by soft ureteroscopy is insufficient.
