【摘要】：目的評價輸尿管軟鏡(FURL)聯(lián)合鈥激光碎石術(shù)同期治療雙側(cè)上尿路結(jié)石患者的安全性和有效性。方法回顧性分析該院2014年9月至2016年11月采用FURL聯(lián)合鈥激光碎石術(shù)同期治療雙側(cè)上尿路結(jié)石患者43例的臨床資料。所有患者術(shù)前行CT掃描,術(shù)中置入FURL,使用200μm光纖,0.8~1.0J/10~20Hz功率進行碎石,配合使用取石網(wǎng)籃,碎石后常規(guī)留置雙側(cè)輸尿管支架管。術(shù)后1d復查KUB或CT,結(jié)石殘留者4~6周后取出輸尿管支架管時再次復查KUB。結(jié)果所有患者均一次性成功置入輸尿管軟鏡,平均手術(shù)時間(101.5±37.2)min,結(jié)石清除率(SFR)為81.4%(35/43)。結(jié)石負荷小于30mm清除率為100.0%,與大于或等于30mm(63.2%)比較差異有統(tǒng)計學意義(P0.05)。術(shù)后1例患者出現(xiàn)發(fā)熱(39.5℃),1例患者出現(xiàn)腎包膜下血腫,無其他嚴重并發(fā)癥發(fā)生,術(shù)后住院時間平均4d。結(jié)論 FURL聯(lián)合鈥激光碎石術(shù)是同期治療雙側(cè)上尿路結(jié)石的有效手段,尤其是對于結(jié)石負荷小于30mm者,具有較高的SFR,安全性良好。
[Abstract]:Objective to evaluate the safety and efficacy of (FURL) combined with holmium laser lithotripsy in the treatment of bilateral upper urinary calculi. Methods the clinical data of 43 patients with bilateral upper urinary calculi treated with FURL combined with holmium laser lithotripsy from September 2014 to November 2016 were retrospectively analyzed. All patients underwent CT scan before operation, 200 渭 m optical fiber was inserted into FURL, 0.8~1.0J/10~20Hz power was used for lithotripsy, and stone basket was used together. After lithotripsy, bilateral ureteral stents were routinely placed. One day after operation, KUB or CT, stones remained in the ureteral stents, and the ureteral stents were removed from the ureteral stents for 4 to 6 weeks. The KUB. was re-examined.
