【摘要】：目的分析因腎臟疾病行經(jīng)皮腎穿刺活檢患者的臨床及病理分型。方法選取2006—2012年中國醫(yī)科大學(xué)附屬盛京醫(yī)院腎臟內(nèi)科住院行經(jīng)皮腎穿刺活檢的遼寧地區(qū)患者1 562例,回顧性分析患者的臨床資料和病理資料。結(jié)果 1 562例經(jīng)皮腎穿刺活檢患者中,原發(fā)性腎小球疾病(PGN)1 222例(78.23%),繼發(fā)性腎小球疾病(SGN)277例(17.73%),腎小管間質(zhì)性疾病(TID)44例(2.82%),遺傳性腎病11例(0.71%),未分類腎病8例(0.51%)。PGN中最常見的病理類型為Ig A腎病475例(38.87%),其次為膜性腎病311例(25.45%)、系膜增生性腎小球腎炎243例(19.89%);SGN中最常見的病理類型為過敏性紫癜性腎炎68例(24.55%),其次為狼瘡性腎炎45例(16.25%)、糖尿病腎病44例(15.88%)。不同年齡患者腎臟病理檢查結(jié)果構(gòu)成比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05);不同性別和年份患者腎臟病理檢查結(jié)果構(gòu)成比較,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。腎穿刺指征分別為腎病綜合征662例(42.38%),腎炎綜合征448例(28.68%)、無癥狀性尿異常223例(14.28%)、急性腎功能不全105例(6.72%)、肉眼血尿59例(3.78%)、慢性腎功能不全53例(3.39%)、其他12例(0.77%)。不同性別患者腎穿刺指征比較,差異無統(tǒng)計(jì)學(xué)意義(P0.05);不同年齡和腎臟病理檢查結(jié)果患者腎穿刺指征比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論腎穿刺活檢患者中,PGN最常見,其中Ig A腎病為最常見病理類型,其次為膜性腎病;SGN以過敏性紫癜性腎炎和狼瘡性腎炎多見。PGN和SGN最常見腎穿刺指征為腎病綜合征和腎炎綜合征,同一種病理類型可以有不同的臨床表現(xiàn)。
[Abstract]:Objective to analyze the clinical and pathological types of patients with renal diseases undergoing percutaneous renal biopsy. Methods from 2006 to 2012, 1 562 patients in Department of Renal Medicine of Shengjing Hospital affiliated to China Medical University were selected for percutaneous renal biopsy in Liaoning province. The clinical and pathological data of the patients were analyzed retrospectively. Results among 1 562 patients with percutaneous renal biopsy, Primary glomerular disease 1 222 cases (78.23%), secondary glomerular disease (SGN) 277 cases (17.73%), tubulointerstitial disease (TID) 44 cases (2.82%), hereditary nephropathy 11 cases (0.71%), unclassified nephropathy 8 cases (0.51%) .PGN: Ig A nephropathy 475 cases (38.87%) There were 311 cases (25.45%) of membranous nephropathy, 243 cases (19.89%) of Mesangial proliferative glomerulonephritis, 68 cases (24.55%) of Henoch-Schonlein purpura nephritis, 45 cases (16.25%) of lupus nephritis and 44 cases (15.88%) of diabetic nephropathy. The composition of renal pathological examination results of different age patients, the difference was statistically significant (P0.05); different sex and year of renal pathological examination results were compared, the difference was not statistically significant (P0.05). The renal puncture indications were nephrotic syndrome in 662 cases (42.38%), nephritis syndrome in 448 cases (28.68%), asymptomatic urinary abnormality in 223 cases (14.28%), acute renal insufficiency in 105 cases (6.72%), gross hematuria in 59 cases (3.78%), chronic renal insufficiency in 53 cases (3.39%), and other 12 cases (0.77%). There was no significant difference in renal puncture indication between different gender patients (P0.05), and there was significant difference in renal puncture indication between different age and renal pathological examination (P0.05). Conclusion Ig A nephropathy is the most common pathological type in renal biopsy patients. Secondly, the most common renal puncture indications of membranous nephropathy were allergic purpura nephritis and lupus nephritis. The most common renal puncture indications were nephrotic syndrome and glomerulonephritis syndrome, and the same pathological type could have different clinical manifestations.
