自體管狀肉芽組織重建兔全周尿道的功能組織學(xué)評估

【摘要】:目的探討應(yīng)用自體皮下誘導(dǎo)生成的管狀肉芽組織重建兔全周尿道的可行性。方法 18只雄性兔分為實驗組與對照組,每組9只,全周游離并切除1.0 cm前尿道,實驗組應(yīng)用自體皮下誘導(dǎo)生成的管狀肉芽組織原位修復(fù)尿道缺損,對照組行單純端端吻合。術(shù)后1、2及6個月行尿道造影、大體觀察、組織學(xué)檢測及Western blotting檢測,動態(tài)評估新尿道通暢度及組織再生。結(jié)果組織學(xué)及透射電鏡檢測顯示,新生管狀肉芽組織富含膠原纖維及肌成纖維細胞。實驗組術(shù)后尿道腔保持通暢,對照組則發(fā)生進行性狹窄。實驗組術(shù)后移植物表面逐漸覆蓋完整的尿路上皮層,上皮下可見排列日趨致密規(guī)則的新生平滑肌束。Western blotting示實驗組隨術(shù)后時間延長,移植物細胞廣譜角蛋白AE1/AE3及α-平滑肌肌動蛋白表達量均逐漸增加,術(shù)后6個月其表達量接近正常尿道。結(jié)論自體皮下誘導(dǎo)生成的管狀肉芽組織可成功修復(fù)兔1.0 cm長前尿道缺損。
[Abstract]:Objective to investigate the feasibility of using autologous subcutaneously induced tubular granulation tissue to reconstruct the peripheral urethra of rabbits. Methods Eighteen male rabbits were divided into experimental group and control group with 9 rabbits in each group. The urethral defects were repaired in situ by autologous subcutaneously induced tubular granulation tissue in the experimental group, and the urethral defects were repaired by end-to-end anastomosis in the control group. Urethrography, gross observation, histological examination and Western blotting examination were performed 1 and 6 months after operation. The patency of new urethra and tissue regeneration were evaluated dynamically. Results histological and transmission electron microscopy showed that the new tubular granulation tissue was rich in collagen fibers and myofibroblasts. The urethral cavity remained patency in the experimental group and progressive stenosis occurred in the control group. The graft surface of the experimental group was gradually covered with the intact urinary cortex after operation, and the neonate smooth muscle bundles with regular arrangement were seen under the subepithelium. Western blotting showed that the experimental group was prolonged with the postoperative time. The expressions of broad-spectrum keratin AE1/AE3 and 偽 -smooth muscle actin in grafts were increased gradually, and were close to normal urethra at 6 months after operation. Conclusion autogenous subcutaneously induced tubular granulation tissue can successfully repair 1.0 cm long anterior urethral defect in rabbits.
