【摘要】：目的:通過Meta分析的方法評(píng)價(jià)磁共振波譜成像對(duì)國(guó)人前列腺癌的診斷價(jià)值。方法:搜集現(xiàn)有國(guó)內(nèi)公開發(fā)表的以手術(shù)或病檢結(jié)果為診斷金標(biāo)準(zhǔn)的關(guān)于評(píng)價(jià)磁共振波譜成像對(duì)國(guó)人前列腺癌診斷價(jià)值的中文文獻(xiàn),按照Meta分析的要求和診斷性試驗(yàn)公認(rèn)的質(zhì)量標(biāo)準(zhǔn)對(duì)所有的納入研究進(jìn)行質(zhì)量評(píng)估,并提取各納入研究的特征信息。采用STATA10.0軟件檢驗(yàn)異質(zhì)性,根據(jù)異質(zhì)性結(jié)果選擇相應(yīng)的效應(yīng)模型,采用MetaDisc1.4軟件計(jì)算出匯總敏感度和特異度,以診斷性試驗(yàn)Meta分析特有的綜合受試者工作特性曲線(SROC)分析方法定性、定量地評(píng)價(jià)該診斷方法的準(zhǔn)確度。結(jié)果:符合納入標(biāo)準(zhǔn)的文獻(xiàn)共10篇,匯總的診斷指標(biāo)和SROC分析均顯示磁共振波譜成像對(duì)國(guó)人前列腺癌的診斷具有較高的準(zhǔn)確度,表示診斷試驗(yàn)準(zhǔn)確度的指標(biāo)匯總敏感度為93%(95%可信區(qū)間0.90~0.96),匯總特異度為92%(95%可信區(qū)間0.89~0.95),AUC=0.9789,Q指數(shù)=0.9351。結(jié)論:匯總目前關(guān)于磁共振波譜成像對(duì)國(guó)人前列腺癌的研究顯示,磁共振波譜成像對(duì)診斷國(guó)人前列腺癌具有較高的敏感度和特異度。
[Abstract]:Objective: to evaluate the diagnostic value of magnetic resonance spectroscopy (MRI) in Chinese prostate cancer by Meta analysis. Methods: the Chinese literature on the evaluation of the value of magnetic resonance spectroscopy (MRI) in the diagnosis of prostate cancer was collected and published in China. According to the requirement of Meta analysis and the accepted quality standard of diagnostic test, the quality of all inclusive studies is evaluated, and the characteristic information of each included study is extracted. The heterogeneity was tested by STATA10.0 software, the corresponding effect model was selected according to the heterogeneity result, and the aggregate sensitivity and specificity were calculated by MetaDisc1.4 software. The diagnostic test Meta was used to analyze the characteristic curve of comprehensive subjects' operating characteristics by (SROC) analysis method qualitatively. The accuracy of the diagnostic method is quantitatively evaluated. Results: there were 10 articles in accordance with the inclusion criteria. The total diagnostic indexes and SROC analysis showed that magnetic resonance spectroscopy imaging had a high accuracy in the diagnosis of prostate cancer in Chinese. The total sensitivity and specificity of diagnostic test were 93% (95% confidence interval 0.90 ~ 0.96) and 92% (95% confidence interval 0.89 ~ 0.95) respectively. Conclusion: magnetic resonance spectroscopy imaging has a high sensitivity and specificity in the diagnosis of Chinese prostate cancer.
