【摘要】：正患者女,68歲。既往反復(fù)發(fā)作哮喘50余年,此次因體檢腹部超聲發(fā)現(xiàn)右腎腫物(大小約8.5cm×7.6cm),腹部CT檢查診斷右腎血管平滑肌脂肪瘤。后于我院泌尿外科行全麻下機(jī)器人輔助腹腔鏡腎部分切除術(shù)(右側(cè))。術(shù)前血分析、肝腎功能正常。手術(shù)順利。術(shù)后6h患者哮喘發(fā)作,給予吸入糖皮質(zhì)激素后癥狀控制。術(shù)后15h患者感右上腹脹痛,觸診右上腹壁存在"捻發(fā)音",輕微壓痛,無(wú)反跳痛。行急診床邊消化系統(tǒng)超聲檢查:右側(cè)腹壁皮下見(jiàn)細(xì)帶狀強(qiáng)回聲(上至劍突下,下至臍下2cm,左至腹中線(xiàn),右至腋后線(xiàn)),后伴寬大聲影,致使肝右葉、部分肝
[Abstract]:The patient is 68 years old. For more than 50 years of recurrent asthma, the right renal mass (8.5cm 脳 7.6cm) was found by abdominal ultrasound, and the right renal angiomyolipoma was diagnosed by abdominal CT. Robo-assisted laparoscopic partial nephrectomy (right) under general anesthesia was performed in our urology department. Preoperative blood analysis showed normal liver and kidney function. The operation went well. Patients with asthma attack 6 hours after the operation were given symptoms control after inhaling glucocorticoid. 15 hours after operation, the patient felt distended pain in the right upper abdomen, and there was "twisted pronunciation" in the palpation of the right epigastric wall, with slight tenderness and no rebound pain.
