一般人群中尿蛋白水平對主要不良心腦血管事件及全因死亡的影響

【摘要】:目的研究一般人群中尿蛋白水平對主要不良心腦血管事件(MACCE)及全因死亡的影響。方法采用前瞻性隊(duì)列研究方法,入選2006年7月至2007年10月參加開灤集團(tuán)健康體檢人群中資料完整者共87292例作為研究對象,平均年齡51.12±12.29歲。采用尿常規(guī)試紙法檢測晨尿,以尿蛋白水平分為3組:尿蛋白陰性組(-)、微量尿蛋白組(±/+)、大量尿蛋白組(≥2+)。應(yīng)用COX比例風(fēng)險(xiǎn)模型分析尿蛋白與MACCE及全因死亡的關(guān)系。結(jié)果平均隨訪6.90±0.72年,隨訪期間共發(fā)生MACCE 3091例,累積發(fā)病率為5.25/千人年;全因死亡事件4087例,累積發(fā)病率為6.84/千人年。3組MACCE累積發(fā)病率分別為4.84/千人年、7.34/千人年、12.06/千人年;全因死亡率分別為6.10/千人年、10.28/千人年、19.30/千人年,差異均具有統(tǒng)計(jì)學(xué)意義(P0.01)。校正其他傳統(tǒng)危險(xiǎn)因素后,與尿蛋白陰性組相比,微量尿蛋白組、大量尿蛋白組發(fā)生MACCE的RR值分別為1.24(95%CI 1.10~1.40)、1.58(95%CI 1.38~1.81),全因死亡的RR值分別為1.41(95%CI 1.27~1.56)、2.15(95%CI 1.93~2.39)。結(jié)論尿常規(guī)顯示的尿蛋白是一般人群MACCE及全因死亡的獨(dú)立危險(xiǎn)因素。
