【摘要】：目的:探討轉(zhuǎn)移性去勢(shì)抵抗性前列腺癌(metastatic castration resistant prostate cancer,m CRPC)患者接受多西他賽+潑尼松治療有效性的影響因素。方法:回顧性分析2010年2月至2015年3月在北京大學(xué)第一醫(yī)院轉(zhuǎn)移性去勢(shì)抵抗性前列腺癌患者接受多西他賽聯(lián)合潑尼松治療有效性的影響因素。研究納入的多西他賽聯(lián)合潑尼松治療的m CRPC患者的臨床資料,對(duì)相關(guān)的臨床因素進(jìn)行單因素分析。結(jié)果:共60例患者接受治療,對(duì)其中33例臨床資料完整的患者進(jìn)行分析�；熡行Фx為化療4個(gè)周期后前列腺特異性抗原(prostate specific antigen,PSA)較基線下降≥50%,且影像學(xué)改變按照實(shí)體腫瘤療效評(píng)價(jià)標(biāo)準(zhǔn)(response evaluation criteria in solid tumors,RECIST)無(wú)進(jìn)展。其化療時(shí)中位PSA為153.4μg/L(6.6~9 952.0μg/L),共20例(60.6%)為治療有效,單因素分析后發(fā)現(xiàn)Gleason評(píng)分較低者化療有效性較差(P=0.034);化療時(shí)存在淋巴結(jié)轉(zhuǎn)移者有效率較高(P=0.032);內(nèi)臟轉(zhuǎn)移者共15例,其中12例(80%)患者治療有效(P=0.041);化療時(shí)出現(xiàn)貧血的患者(HGB≤120 g/L)共10例,化療有效者3例(30%,P=0.024)。結(jié)論:對(duì)于m CRPC患者,初發(fā)時(shí)Gleason評(píng)分≤7、診斷時(shí)合并淋巴結(jié)轉(zhuǎn)移、內(nèi)臟轉(zhuǎn)移以及化療時(shí)存在貧血是化療能否有效的預(yù)測(cè)因素。
[Abstract]:Objective: to investigate the factors influencing the efficacy of docetaxel prednisone in metastatic ovariectomized prostate cancer (metastatic castration resistant prostate cancer-CRPC). Methods: the factors influencing the efficacy of docetaxel combined with prednisone in metastatic ovariectomized prostate cancer patients in Peking University first Hospital from February 2010 to March 2015 were analyzed retrospectively. The clinical data of m CRPC patients treated with docetaxel combined with prednisone were analyzed by univariate analysis. Results: a total of 60 patients were treated and 33 patients with complete clinical data were analyzed. The effective definition of chemotherapy is that the prostate specific antigen (prostate specific antigen-PSA) decreases more than 50% compared with baseline after 4 cycles of chemotherapy, and no progress has been made in imaging changes according to the criteria for evaluating the efficacy of solid tumors, (response evaluation criteria in solid tumors / RECIST. At the time of chemotherapy, the median PSA was 153.4 渭 g / L (6.6 渭 g / L, 9 952.0 渭 g / L), and 20 cases (60.6%) were effective.
