【摘要】：目的探討MRI在氬氦刀冷凍治療前列腺癌術(shù)后療效評價(jià)中的價(jià)值。方法系統(tǒng)回顧2012年3月—2014年10月16例超聲引導(dǎo)下氬氦刀冷凍治療前列腺癌的臨床及影像資料,分析術(shù)前術(shù)后實(shí)驗(yàn)室指標(biāo)的變化,對比術(shù)前術(shù)后MRI資料,觀察消融效果,包括有無術(shù)后殘留、轉(zhuǎn)移等。結(jié)果術(shù)后1個(gè)月MRI提示所有患者冷凍消融范圍滿意,均表現(xiàn)為長T2信號,DWI及DCE-MRI未見明顯異常復(fù)發(fā),前列腺特異性抗原(PSA)輕度下降。術(shù)后6個(gè)月MRI提示前列腺明顯縮小,PSA明顯下降,未見明顯復(fù)發(fā)征象。術(shù)后無尿道壞死、尿道狹窄、尿道直腸瘺發(fā)生。1例患者隨訪6個(gè)月時(shí)因其他原因死亡。結(jié)論 MRI在評價(jià)氬氦刀低溫冷凍治療前列腺癌消融范圍、術(shù)后復(fù)發(fā)或轉(zhuǎn)移等療效方面具有較好的臨床應(yīng)用價(jià)值。
[Abstract]:Objective to evaluate the value of MRI in the treatment of prostate cancer after cryotherapy with argon-helium knife. Methods the clinical and imaging data of 16 patients with prostate cancer treated by cryotherapy with argon-helium knife guided by ultrasound from March 2012 to October 2014 were reviewed systematically. The changes of laboratory indexes before and after operation were analyzed. The MRI data before and after operation were compared and the ablation effect was observed. Including postoperative residue, metastasis and so on. Results one month after operation, MRI showed that the range of cryoablation was satisfactory in all the patients, showing that there was no obvious recurrence on DWI and DCE-MRI with long T 2 signal intensity, and a slight decrease in prostate specific antigen (PSAs). Six months after operation, MRI showed that the prostate was significantly reduced and PSA was significantly decreased, but no recurrence was found. There was no urethral necrosis, urethral stricture and urethrorectal fistula. Conclusion MRI has a good clinical value in evaluating the range of ablation, recurrence or metastasis of prostate cancer by cryotherapy with argon-helium knife.
