【摘要】：目的:比較經(jīng)腹途徑與經(jīng)腹膜外途徑機器人輔助腹腔鏡下根治性前列腺切除術(shù)的圍手術(shù)期結(jié)果。方法:回顧性分析南京醫(yī)科大學(xué)第一附屬醫(yī)院2016年6月—2016年9月53例行機器人輔助腹腔鏡下根治性前列腺切除術(shù)前列腺癌患者的臨床資料,其中經(jīng)腹途徑41例,年齡56~80歲,平均70歲;經(jīng)腹膜外途徑12例,年齡59~78歲,平均68歲。比較兩組患者圍手術(shù)期情況。結(jié)果:53例機器人輔助腹腔鏡下根治性前列腺切除術(shù)均順利完成,無中轉(zhuǎn)開放手術(shù)者。比較經(jīng)腹和經(jīng)腹膜外兩組年齡[(70.4±6.0)歲vs.(68.3±6.8)歲]、BMI[(23.7±2.5)kg/m~2vs.(24.1±3.2)kg/m~2]、術(shù)前PSA[(22.2±21.6)ng/mL vs.(14.9±9.0)ng/mL]、Gleason評分[(7.5±0.7)分vs.(7.3±0.7)分]比較差異均無統(tǒng)計學(xué)意義(P0.05)。經(jīng)腹組和經(jīng)腹膜外組的總手術(shù)時間[(223.4±26.8)min vs.(186.5±16.6)min]、裝機時間[(23.6±4.9)min vs.(27.9±5.5)min]、術(shù)后腸道恢復(fù)排氣時間[(2.4±0.6)d vs.(1.9±0.6)d]、術(shù)后腸道恢復(fù)排便時間[(3.6±0.5)d vs.(3.2±0.6)d]差異均有統(tǒng)計學(xué)意義(P0.05)。兩組的術(shù)中出血量、術(shù)后雙側(cè)盆腔引流管留置時間、術(shù)后下床活動時間、術(shù)后住院時間、術(shù)后導(dǎo)尿管留置時間比較差異均無統(tǒng)計學(xué)意義(P0.05)。結(jié)論:與經(jīng)腹途徑比較,經(jīng)腹膜外途徑機器人輔助腹腔鏡下根治性前列腺切除術(shù)手術(shù)時間短,術(shù)后腸道恢復(fù)時間快,在術(shù)中出血量、術(shù)后雙側(cè)盆腔引流管留置時間、術(shù)后下床活動時間、術(shù)后住院時間、術(shù)后導(dǎo)尿管留置時間等方面無明顯差異。
[Abstract]:Objective: to compare the perioperative results of laparoscopic radical prostatectomy with transabdominal approach and extraperitoneal approach. Methods: the clinical data of 53 patients with prostatic cancer undergoing radical prostatectomy with robotic assisted laparoscopic prostatectomy from June 2016 to September 2016 in the first affiliated Hospital of Nanjing Medical University were retrospectively analyzed. The mean age was 70 years, and the mean age was 68 years (59 78 years) in 12 patients with extraperitoneal approach. The perioperative period was compared between the two groups. Results all 53 cases of Robotic assisted laparoscopic radical prostatectomy were successfully completed and there was no conversion to open prostatectomy. There was no significant difference between the two groups (70.4 鹵6.0 years old, vs.(68.3 鹵6.8 years old) and preoperative PSA (22.2 鹵21.6)ng/mL vs.(14.9 鹵9.0)ng/mL), vs.(7.3 鹵0.7 (7.5 鹵0.7). There were significant differences in total operation time (223.4 鹵26.8)min vs.(186.5 鹵16.6)min), installation time (23.6 鹵4.9)min vs.(27.9 鹵5.5)min), postoperative bowel recovery time (2.4 鹵0.6d vs.(1.9 鹵0.6d) and postoperative bowel recovery time (3.6 鹵0.5d vs.(3.2 鹵0.6d) between the transabdominal group and the extraperitoneal group (P 0.05). There was no significant difference between the two groups in the amount of intraoperative bleeding, the time of bilateral pelvic drainage tube indwelling, the time of getting out of bed, the time of hospitalization and the time of catheter indwelling after operation (P 0.05). Conclusion: compared with the transabdominal approach, the operation time of robot-assisted laparoscopic prostatectomy is shorter, the recovery time of intestinal tract is faster, the amount of blood lost during the operation and the time of indwelling the bilateral pelvic drainage tube. There was no significant difference in postoperative time of bed movement, postoperative hospitalization time and postoperative catheter indwelling time.
