【摘要】：目的:探討再次心臟瓣膜置換術(shù)后急性腎功能衰竭發(fā)生的危險因素。方法:回顧272例再次心臟瓣膜置換術(shù)患者的臨床基本情況、術(shù)前腎功能、手術(shù)方式、體外循環(huán)參數(shù)等相關(guān)臨床資料,通過多因素Logistic回顧分析術(shù)后發(fā)生急性腎功能損傷的危險因素。結(jié)果:272例患者中術(shù)后發(fā)生急性腎功能衰竭12例。多因素logistic回歸分析顯示術(shù)前合并其他系統(tǒng)疾病(OR=9.104,P=0.029)、急診手術(shù)(OR=90.998,P=0.000)、女性患者(OR=46.449,P=0.016)、術(shù)前左心室內(nèi)徑60mm(OR=0.114,P=0.041)是導(dǎo)致患者術(shù)后急性腎功能衰竭發(fā)生的危險因素。結(jié)論:急性腎功能衰竭是再次心臟瓣膜術(shù)后最危險的并發(fā)癥之一,術(shù)前積極治療患者其他系統(tǒng)疾病、選擇合理的手術(shù)時機,術(shù)中盡量縮短體外循環(huán)轉(zhuǎn)流時間,術(shù)后加強對患者腎功能變化的監(jiān)測有助于降低腎功能衰竭的發(fā)生率。
[Abstract]:Objective: to investigate the risk factors of acute renal failure after valve replacement. Methods: the clinical data of 272 patients undergoing cardiac valve replacement were reviewed, including renal function before operation, operation mode, parameters of cardiopulmonary bypass, and so on. The risk factors of acute renal function injury after operation were analyzed retrospectively by multivariate Logistic. Results 12 cases of acute renal failure occurred after operation in 272 cases. Multivariate logistic regression analysis showed that the risk factors of postoperative acute renal failure were OR9.104 P0.029, emergency operation 90.998P0. 000, female patients 46.449 P0.016, preoperative left ventricular diameter 60mm or 0.114 P0.041), multivariate logistic regression analysis showed that the risk factors of acute renal failure were preoperatively combined with other systemic diseases (OR9.104), emergency operation (90.998U), female patients (46.449), preoperative left ventricular internal diameter (60mm) and left ventricular internal diameter (60mm) were the risk factors of postoperative acute renal failure (ARF). Conclusion: acute renal failure is one of the most dangerous complications after heart valve reoperation. Strengthening the monitoring of renal function after operation can reduce the incidence of renal failure.
