【摘要】：目的探討IgA腎病患者臨床表現(xiàn)、病理特點及其相關(guān)性。方法對我院收治的153例IgA腎病患者相關(guān)資料進(jìn)行綜合性回顧性分析,入選患者均進(jìn)行過腎活檢病理確診為IgA腎病,分析IgA腎病臨床表現(xiàn)、病理類型等。結(jié)果 40例患者為有誘因發(fā)病,其中22例上呼吸道感染、3例妊娠、7例疲勞、4例腸道感染,1例由于海鮮引起過敏,1例腰部扭傷,2例手術(shù)原因;Ι級病理患者中6例為蛋白尿聯(lián)合血尿型,Ⅱ級病理中7例單純血尿型,10例蛋白尿聯(lián)合血尿型;Ⅲ級病理中,20例蛋白尿聯(lián)合血尿型;Ⅳ級病理中27例蛋白尿聯(lián)合血尿型;Ⅴ級病理中5例單純血尿型,5例蛋白尿聯(lián)合血尿型;免疫復(fù)合IgA中有12例,占7.8%,IgA+Ig M占13.7%;IgA+C3占17.0%;IgA+Ig G+Ig M占5.2%;IgA+Ig G+C3占7.2%;IgA+Ig M+C3占32.7%;IgA+Ig G+Ig M+C3占17%。結(jié)論 IgA腎病在感染誘因下發(fā)病率較高,患者發(fā)病后臨床表現(xiàn)以蛋白尿聯(lián)合血尿型為主;同時,腎衰竭型比例出現(xiàn)上升趨勢,且患者治療預(yù)后不佳,臨床上應(yīng)該根據(jù)患者病理結(jié)果進(jìn)行積極有效治療,降低腎衰竭發(fā)生率。
[Abstract]:Objective to investigate the clinical manifestations, pathological features and correlation of IgA nephropathy. Methods the data of 153 patients with IgA nephropathy in our hospital were analyzed retrospectively. All the patients were diagnosed as IgA nephropathy by renal biopsy, and the clinical manifestations and pathological types of IgA nephropathy were analyzed. Results there were 40 patients with predisposing disease. There were 22 cases of upper respiratory tract infection 3 cases of pregnancy, 7 cases of tiredness, 4 cases of intestinal infection, 1 case of seafood allergy, 1 case of lumbar sprain, 2 cases of surgical causes, 6 cases of proteinuria combined with hematuria, 6 cases of grade I pathology.
