【摘要】：IgG4相關性疾病是以血清IgG4升高、組織IgG4漿細胞浸潤以及纖維性病變?yōu)樘卣鞯亩嘞到y(tǒng)疾病,可累及胰腺、腎臟、唾液腺等多種器官,累及腎臟時稱為IgG4相關性腎病,主要表現(xiàn)為IgG4相關性腎小管間質(zhì)性腎炎,累及腎小球時以膜性腎病多見。而特發(fā)性膜性腎病的免疫復合物沉積也以IgG4為主,在臨床上區(qū)分IgG4相關性疾病繼發(fā)的膜性腎病與特發(fā)性膜性腎病非常重要。本文從發(fā)病機制、臨床表現(xiàn)、診斷及治療等方面探討IgG4相關性腎病與特發(fā)性膜性腎病的區(qū)別,為臨床的診治提供一定的理論基礎。
[Abstract]:IgG4 associated disease is a multisystem disease characterized by the elevation of serum IgG4, the infiltration of IgG4 plasma cells and fibrous lesions. It can involve many organs, such as pancreas, kidney, salivary gland and so on. When the kidney is involved, it is called IgG4 associated nephropathy. IgG4 associated tubulointerstitial glomerulonephritis was the main manifestation, and membranous nephropathy was more common in glomeruli. The immune complex deposition of idiopathic membranous nephropathy is also dominated by IgG4. It is very important to distinguish membranous nephropathy from idiopathic membranous nephropathy in clinic. This paper discusses the differences between IgG4 associated nephropathy and idiopathic membranous nephropathy from the aspects of pathogenesis, clinical manifestation, diagnosis and treatment, and provides a theoretical basis for clinical diagnosis and treatment.
