【摘要】：目的：探討經(jīng)腎造瘺管注射血凝酶用于預(yù)防與治療微創(chuàng)經(jīng)皮腎(mPCNL)術(shù)后出血并發(fā)癥的效果。 方法：選取2013年6月到2014年1月間中南大學(xué)湘雅醫(yī)院泌尿外科行mPCNL的226例病人。所有病例隨機分為四組：6U血凝酶組、4U血凝酶組、生理鹽水組和對照組。比較四組患者術(shù)后前3天出血量、術(shù)后發(fā)熱率及并發(fā)癥率。采用SPSS17.0統(tǒng)計軟件包以方差分析(one-way ANOVA)對比四組間數(shù)據(jù)。 結(jié)果：有226例患者納入本實驗,各組患者術(shù)前一般資料無統(tǒng)計學(xué)差異。術(shù)后前3天出血量方面,4U血凝酶組和6U血凝酶組的術(shù)后前三天出血量明顯低于生理鹽水組和對照組,差異具有統(tǒng)計學(xué)意義(P0.05)。4U血凝酶組和6U血凝酶組術(shù)后前三天出血量無明顯差異(P0.05)。生理鹽水組和對照組術(shù)后前三天出血量無明顯差異(P0.05)。各組患者在術(shù)后發(fā)熱率及并發(fā)癥率方面無差異。 結(jié)論：mPCNL術(shù)后由造瘺管注射血凝酶可以有效地減少術(shù)后出血,且該操作并不增加術(shù)后發(fā)熱率及并發(fā)癥率。
[Abstract]:Objective: to investigate the effect of hemagglutinin injection through renal fistula tube for the prevention and treatment of bleeding complications after minimally invasive percutaneous nephrostomy. Methods: 226 patients with mPCNL in Xiangya Hospital of Central South University from June 2013 to January 2014 were selected. All the patients were randomly divided into four groups: 1% 6 U hemocoagulase group, 4 U hemocoagulase group, normal saline group and control group. The bleeding volume, postoperative fever rate and complication rate were compared between the four groups. The four groups of data were compared with one-way ANOVA by SPSS17.0 software package. Results: 226 patients were included in this study. In the first 3 days after operation, the volume of blood loss in the 4U and 6U hemagglutinin groups was significantly lower than that in the saline group and the control group. There was no significant difference in the amount of blood loss between P0.05U group and 6U group in the first three days after operation. There was no significant difference in blood loss between the saline group and the control group on the first three days after operation. There was no difference in postoperative fever rate and complication rate in all groups. Conclusion injection of hemocoagulase into the fistula tube can effectively reduce postoperative bleeding and does not increase the rate of postoperative fever and complications.
