【摘要】：目的：目前篩選前列腺癌的最常用的腫瘤標(biāo)記物是前列腺特異性抗原（PSA），但是由于PSA特異性及對(duì)前列腺癌的早期診斷價(jià)值仍然存在著爭議，而最近發(fā)現(xiàn)非編碼前列腺癌基因3（PCA3）是前列腺癌（PCa）診斷最具特異性的生物標(biāo)志物。雖然在前列腺按摩后的尿液診斷中已經(jīng)驗(yàn)證了其臨床價(jià)值，但是很少有研究確定其在外周血淋巴細(xì)胞（PBL）中的診斷價(jià)值。本研究的目的是檢測血液中的PCA3表達(dá)，以此作為一種診斷工具，提供改進(jìn)前列腺癌診斷的方法。方法：收集2012年6月至2013年11月期間桂北、桂中地區(qū)大型醫(yī)院住院患者外周血標(biāo)本和前列腺組織標(biāo)本，其中包括28例前列腺癌（PCa）患者的外周血液標(biāo)本與組織標(biāo)本、34例良性前列腺增生（BPH）患者的外周血液標(biāo)本與組織標(biāo)本以及31例作為正常對(duì)照組的非前列腺疾病患者的外周血標(biāo)本。通過RT-PCR檢測患者外周血和前列腺組織的PCA3基因的mRNA的表達(dá)。然后，分析不同分組（前列腺癌組、良性前列腺增生組、正常對(duì)照組）血液和組織中PCA3的RNA表達(dá)有無差異。分析前列腺癌患者PCA3基因的mRNA的表達(dá)與Gleason評(píng)分和腫瘤TNM分期的關(guān)系，以及其表達(dá)聯(lián)合前列腺特異性抗原（PSA）水平對(duì)前列腺癌診斷參數(shù)的影響。結(jié)果：經(jīng)檢測分析，前列腺癌組與良性前列腺增生組、對(duì)照組的PCA3的表達(dá)有顯著性差異（P0.005），，前列腺癌（PCa）組外周血和組織中PCA3的mRNA表達(dá)明顯高于對(duì)照組和良性前列腺增生（BPH）組。而良性前列腺增生（BPH）組與對(duì)照組無顯著性差異（P0.05）。血液中的PCA3診斷前列腺癌表現(xiàn)出96.9%的特異性和32.1%的敏感性，聯(lián)合檢測前列腺組織中的PCA3顯著改善了該診斷參數(shù)。同時(shí)，檢測血液中PCA3的表達(dá)聯(lián)合PSA≥10ng/ml顯示出95.4%的特異性和60.7%的敏感性。結(jié)論：1．外周血和組織中PCA3基因的mRNA表達(dá)是前列腺癌診斷的特異性指標(biāo)，檢測患者血液中的PCA3的mRNA，可作為篩選前列腺癌的腫瘤標(biāo)記物。2．檢測外周血PCA3的mRNA表達(dá)聯(lián)合PSA≥10ng/mL，可進(jìn)一步提高前列腺癌診斷的敏感性和特異性，并有效防止誤診。
[Abstract]:Objective : To investigate the expression of PCA3 mRNA in peripheral blood and prostate tissues of patients with prostate cancer ( PCa ) . The results showed that the expression of PCA3 mRNA in peripheral blood and tissues of patients with benign prostatic hyperplasia ( PCa ) was significantly higher than that of control group and benign prostatic hyperplasia ( BPH ) . The mRNA expression of the PCA3 gene in peripheral blood and tissues is a specific marker for the diagnosis of prostate cancer , and the mRNA of the PCA3 mRNA in the blood of the patient can be detected as a tumor marker for screening prostate cancer .
