【摘要】：目的了解雙J管管壁結石形成的基本情況,探討分析雙J管管壁結石形成的影響因素。方法納入2014年2~7月在我科因上尿路結石接受輸尿管軟鏡聯合鈥激光碎石術的患者共84例,收集相關臨床資料,使用電腦體式顯微鏡觀測雙J管管壁結石形成情況,對臨床資料與管壁結石形成之間的關系采用t檢驗、χ2檢驗、Fisher確切概率法、logistic回歸進行統(tǒng)計學分析。結果雙J管平均留置時間(17.0±6.0)d,67例(79.8%)雙J管形成管壁石膜。石膜形成組較未形成石膜組更年輕〔(44.9±11.5)歲vs.(54.4±12.6)歲,P=0.004〕;尿蛋白陽性患者的管壁石膜形成率更高〔62/73(84.9%)vs.5/11(45.5%),P=0.002〕;尿路感染陽性者較陰性者石膜形成率高〔26/28(92.9%)vs.41/56(73.2%),P=0.035〕;血尿陽性者較陰性者石膜形成率高〔67/80(83.8%)vs.0/4,P=0.001〕。不同性別、雙J管留置時間、血清鈣、無機磷、尿酸、尿pH、結石類型之間石膜形成率差異無統(tǒng)計學意義(P0.05)。取5例雙J管管壁結石進行結石成分分析,發(fā)現與原發(fā)結石成分完全一致。Logistic回歸發(fā)現石膜形成與低齡和尿蛋白相關(P0.05),與血尿和尿路感染無關(P0.05)。結論影響管壁石垢形成的因素眾多,尿蛋白陽性和低齡是雙J管表面結石形成的重要危險因素。
[Abstract]:Objective to understand the formation of double J tube wall stones and analyze the influencing factors of double J tube wall stone formation. Methods A total of 84 patients with upper urinary calculi underwent ureteroscopy combined with holmium laser lithotripsy from February to July 2014 were included. The clinical data were collected and the formation of double J tube wall stones was observed by computer microscope. T test and 蠂 2 test were used to analyze the relationship between the clinical data and the formation of wall stones. The Fisher exact probability method was used to analyze the relationship between the clinical data and the formation of wall stones. Results the mean indwelling time of double J tube was 17.0 鹵6.0 days and 67 cases (79.8%) of double J tube formed wall stone membrane. The stone membrane forming group was younger than the unformed stone membrane group (44.9 鹵11.5) years old vs.(54.4 鹵12.6years old, the uroprotein positive group had a higher formation rate of the wall stone membrane (P 0.004), and the formation rate of the stone membrane was higher than that of the patients with positive urinary tract infection (2628% 92.9% vs 41% 56 / 73.2P 0.035); the patients with positive urinary tract infection had higher formation rate of stone membrane than those with negative stone membrane formation group (P 0.002), and the patients with positive urinary tract infection had higher stone membrane formation rate than those with positive urinary tract infection (P 0.002), and those with positive urinary tract infection had higher stone membrane formation rate than those with positive urinary tract infection (vs.(54.4 鹵12.6) years old. The rate of formation of stone membrane in hematuria positive group was higher than that in negative group. The rate of formation of stone membrane was 67 / 80 / 83.8 / 0 / 0 / 4 / 0 / 0. 001g. Different sex, double J tube retention time, serum calcium, inorganic phosphorus, uric acid, urine pH, stone formation rate between the type of stone there was no significant difference (P 0.05). Five cases of double J tube wall stones were analyzed for stone composition. The results of logistic regression showed that the formation of stone membrane was associated with low age and urine protein (P0.05), but not with hematuria and urinary tract infection (P0.05). Conclusion there are many factors influencing the formation of wall stone. Urine protein positive and young age are important risk factors for the formation of double J tube surface stones.
