【摘要】：目的探討維持性腹膜透析患者殘腎功能與血管內(nèi)皮功能的相關(guān)性。方法選取本院腹透中心維持性腹透患者101例進(jìn)行橫斷面研究,平均年齡(47.4±12.3)歲,男55例,平均透析齡(23.4±19.7)個(gè)月。收集相關(guān)臨床資料及實(shí)驗(yàn)室檢查數(shù)據(jù),計(jì)算殘腎腎小球?yàn)V過率(GFR)以評(píng)估殘腎功能,使用B超檢測(cè)肱動(dòng)脈血管舒張功能(FMD)以評(píng)估血管內(nèi)皮功能。根據(jù)FMD和殘腎GFR進(jìn)行分組,比較不同分組患者相關(guān)指標(biāo)的差別,用Pearson相關(guān)性分析法分析FMD與殘腎功能的相關(guān)性,用多因素logistic分析法分析腹透患者發(fā)生內(nèi)皮功能紊亂的危險(xiǎn)因素。結(jié)果正常FMD組患者殘腎GFR顯著高于低FMD組患者,而高殘腎GFR組患者FMD顯著高于低殘腎GFR組患者。相關(guān)性分析顯示殘腎GFR、殘腎Kt/V及總Kt/V與FMD呈正相關(guān),年齡、透析齡、脈壓、超敏C反應(yīng)蛋白(hs-CRP)及4 h D/Pcr與FMD呈負(fù)相關(guān)。多因素logistic分析顯示殘腎GFR、年齡及l(fā)og hs-CRP是FMD的獨(dú)立危險(xiǎn)因素。結(jié)論腹透患者血管內(nèi)皮功能與殘腎功能相關(guān),殘腎功能的動(dòng)態(tài)評(píng)估有助于預(yù)測(cè)血管內(nèi)皮功能的變化。
[Abstract]:Objective to investigate the relationship between residual renal function and vascular endothelial function in maintenance peritoneal dialysis patients. Methods A cross-sectional study was conducted in 101 patients with central maintenance dialysis in our hospital. The mean age was 47.4 鹵12.3 years old, 55 males, with an average dialysis age of 23.4 鹵19.7months. Relevant clinical data and laboratory data were collected to evaluate residual renal function by calculating glomerular filtration rate (GFR) and vascular diastolic function (FMDD) of brachial artery were measured by B-ultrasound to evaluate vascular endothelial function. According to FMD and residual kidney GFR, we compared the difference of relative indexes between different groups. The correlation between FMD and residual renal function was analyzed by Pearson correlation analysis, and the risk factors of endothelial dysfunction in patients with abdominal dialysis were analyzed by multivariate logistic analysis. Results GFR of residual kidney in normal FMD group was significantly higher than that in low FMD group, while FMD in high residual GFR group was significantly higher than that in low residual renal GFR group. Correlation analysis showed that GFR, Kt/V and total Kt/V were positively correlated with FMD. Age, dialysis age, pulse pressure, hypersensitive C-reactive protein hs-CRP) and 4 h D/Pcr were negatively correlated with FMD. Multivariate logistic analysis showed that GFR, age and log hs-CRP were independent risk factors for FMD. Conclusion Endothelial function is related to residual renal function in peritoneal dialysis patients. Dynamic evaluation of residual renal function is helpful to predict the changes of vascular endothelial function.
