【摘要】：目的探討輸尿管鏡碎石術(shù)并發(fā)腎臟破裂的可能原因及應(yīng)對策略。方法收集該院行輸尿管鏡碎石術(shù)并發(fā)嚴(yán)重腎破裂的4例患者資料,分析其圍術(shù)期臨床資料,總結(jié)診治經(jīng)驗(yàn)。結(jié)果 4例患者出現(xiàn)患側(cè)腎破裂大出血,3例為腎包膜下血腫,1例重度積液患者出血填塞集合系統(tǒng),4例患者均通過保守治療痊愈。結(jié)論輸尿管鏡碎石術(shù)并發(fā)腎臟破裂是一種少見但嚴(yán)重的并發(fā)癥,同術(shù)前腎皮質(zhì)的厚度和腎臟積液程度關(guān)系密切,另外泌尿系感染、術(shù)中沖水壓力及熟練程度同腎臟損傷的發(fā)生也有一定的相關(guān)性。
[Abstract]:Objective to explore the possible causes and countermeasures of renal rupture after ureteroscopic lithotripsy. Methods the data of 4 cases of ureteroscopic lithotripsy complicated with severe renal rupture were collected, the clinical data during perioperative period were analyzed, and the experience of diagnosis and treatment was summarized. Results there were 4 cases of massive hemorrhage of the affected side of kidney rupture. 3 cases were subcapsular hematoma and 1 case was severe effusion. All the 4 cases were cured by conservative treatment. Conclusion Renal rupture complicated by ureteroscopic lithotripsy is a rare but serious complication, which is closely related to the thickness of renal cortex and the degree of renal effusion, and urinary tract infection. Intraoperative flush pressure and proficiency were also associated with renal injury.
