【摘要】：目的:分析部分型圓頭精子癥患者精液常規(guī)參數(shù)及精子形態(tài)。方法:選取2013年1月至2016年5月100例部分型圓頭精子癥患者作為病例組,對照組為非圓頭精子癥的不育患者180例。根據(jù)精液中圓頭精子所占比例不同將病例組分成5組:1組(25%~40%)、2組(41%~55%)、3組(56%~70%)、4組(71%~85%)、5組(86%~99%),根據(jù)WHO《人類精液檢查與處理實驗室手冊》第5版標(biāo)準(zhǔn)對其進(jìn)行精液常規(guī)分析,包括精子濃度、活動率、前向運(yùn)動精子百分率,及對不同畸形精子進(jìn)行計數(shù)并計算畸形精子指數(shù)(TZI)及精子畸形指數(shù)(SDI)。結(jié)果:病例組與對照組的精子活動率[(35.76±24.88)%vs(62.03±10.20)%]、前向運(yùn)動精子百分率[(26.11±20.39)%vs(45.62±6.87)%]、正常形態(tài)精子百分率[(1.45±1.45)%vs(5.98±2.21)%]和SDI(1.33±0.11 vs1.27±0.57)相比差異均有統(tǒng)計學(xué)意義(P均0.01),兩組年齡[(29.82±4.90)歲vs(30.33±3.59)歲]、精子濃度[(46.01±40.38)×10~6/ml vs(54.00±25.15)×10~6/ml]和TZI(1.35±0.11 vs 1.34±0.54)比較差異均無統(tǒng)計學(xué)意義(P均0.05);病例組各組精子活動率、前向運(yùn)動精子百分率、正常形態(tài)精子百分率、TZI和SDI均有統(tǒng)計學(xué)差異(P均0.01),而年齡和精子濃度均無統(tǒng)計學(xué)差異(P均0.05)。隨著圓頭精子比例的升高,精子頭部形態(tài)由異質(zhì)性偏向于均一性。結(jié)論:精液中不同比例的圓頭精子與精液常規(guī)參數(shù)和形態(tài)學(xué)指標(biāo)密切相關(guān),可對不育患者輔助生殖技術(shù)的選擇和受精率的預(yù)測起一定的提示作用。
[Abstract]:Objective: to analyze the semen routine parameters and sperm morphology in patients with partial round head spermatozoa. Methods: from January 2013 to May 2016, 100 patients with partial round head spermatospermia were selected as the case group, and 180 patients with non-round head spermatozoa as the control group. According to the percentage of round head spermatozoa in semen, the case group was divided into 5 groups: 1 group (25%) and 25% group (2 groups). According to the standard of the fifth edition of WHO, the semen routine analysis, including sperm concentration, was carried out on the basis of the standard of the fifth edition of WHO.
