【摘要】：目的探討經(jīng)直腸前列腺穿刺活檢后經(jīng)尿道前列腺電切術(shù)(transurethral resection of prostate,TURP)的時(shí)機(jī)。方法 2013年9月~2015年9月,經(jīng)直腸前列腺穿刺活檢后行TURP 60例,按時(shí)間先后分為A、B組各30例,2組年齡、前列腺特異抗原(prostate specific antigen,PSA)、前列腺體積、血紅蛋白及國(guó)際前列腺癥狀評(píng)分(international prostate symptom score,IPSS)無(wú)統(tǒng)計(jì)學(xué)差異。A、B組分別于穿刺后1周及4周行TURP,記錄手術(shù)時(shí)間、切除前列腺重量、術(shù)中失血量、術(shù)后膀胱沖洗時(shí)間及術(shù)后3個(gè)月IPSS。結(jié)果 B組手術(shù)時(shí)間、術(shù)中出血量、術(shù)后膀胱沖洗時(shí)間及術(shù)后3個(gè)月IPSS均顯著低于A組[(58.3±6.0)min vs.(62.0±3.3)min,t=2.952,P=0.005;(154.1±15.8)ml vs.(167.4±29.5)ml,t=2.181,P=0.035;(19.2±0.8)h vs.(20.6±2.3)h,t=3.034,P=0.004;(18.3±2.5)分vs.(20.3±2.0)分,t=3.419,P=0.001],切除前列腺重量明顯高于A組[(37.1±4.0)g vs.(33.3±7.8)g,t=-2.341,P=0.024]。結(jié)論經(jīng)直腸前列腺穿刺活檢術(shù)后4周再行TURP,可以顯著增加切除前列腺重量,減少術(shù)中出血量,縮短手術(shù)時(shí)間及術(shù)后膀胱沖洗時(shí)間,更顯著地改善排尿癥狀。如無(wú)特殊情況,建議穿刺后4周行TURP。
[Abstract]:Objective to investigate the timing of transurethral resection of prostate transurethral transurethral resection (TURP) after transrectal prostate biopsy. Methods from September 2013 to September 2015, 60 patients with TURP were divided into two groups: group A (n = 30) and group B (n = 30). There was no statistical difference in hemoglobin and international prostate symptom score. TURP was performed in group A at 1 and 4 weeks after puncture, respectively.
