【摘要】：目的:探討機(jī)器人輔助腹腔鏡前列腺癌根治術(shù)(robot-assisted laparoscopic prostatectomy,RALP)治療復(fù)雜前列腺癌病例的初步經(jīng)驗(yàn)。方法:回顧性分析中南大學(xué)湘雅三醫(yī)院2015年10月至11月行RA LP的4例復(fù)雜前列腺癌患者的臨床和病理資料。4例患者,年齡58~70歲,均接受過經(jīng)尿道等離子鏡下前列腺癌剜除術(shù)和內(nèi)分泌治療。結(jié)果:4例患者手術(shù)均獲成功,基線前列腺特異抗原(prostate specific antigen,PSA)6.04~70.15 ng/m L,臨床分期T2b~T3b,手術(shù)時間180~360 min,術(shù)中出血量50~250 m L,均未輸血,手術(shù)切緣均為陰性。結(jié)論:雖然經(jīng)尿道前列腺手術(shù)史和內(nèi)分泌治療史會增加手術(shù)難度,RA LP仍可用于此類復(fù)雜前列腺癌病例的治療。
[Abstract]:Objective: to investigate the preliminary experience of robot-assisted laparoscopic prostatectomy (RALP) in the treatment of complex prostate cancer.Methods: the clinical and pathological data of 4 patients with complicated prostate cancer, aged 5870 years, who were treated with RA LP from October to November 2015 in Xiangya third Hospital, Central South University, were retrospectively analyzed.All patients underwent transurethral plasma-assisted enucleation of prostate cancer and endocrine therapy.Results all of the 4 patients were successfully operated. The baseline prostate specific antigenation was 6.04 ~ 70.15 ng/m / L, the clinical stage was T _ 2b ~ T _ 3B, the operation time was 180 ~ 360 min, the blood loss was 50 ~ 250m / L, no blood transfusion was found, and the surgical margin was negative.Conclusion: although the history of transurethral prostate surgery and endocrine therapy may increase the surgical difficulty, RA LP can still be used in the treatment of this kind of complicated prostate cancer.
