血液灌流聯(lián)合序貫透析對(duì)老年終末期糖尿病腎病患者炎癥介質(zhì)及氧化應(yīng)激的影響

【摘要】:目的研究血液灌流聯(lián)合序貫透析對(duì)老年終末期糖尿病腎病(DN)患者炎癥介質(zhì)及氧化應(yīng)激的影響。方法終末期DN患者92例根據(jù)治療方法不同分為對(duì)照組和觀察組。觀察組每周2次常規(guī)血透同時(shí)接受每周1次血液灌流聯(lián)合序貫透析治療,對(duì)照組每周進(jìn)行3次血液透析。評(píng)估兩組患者血糖水平、腎功能、炎性介質(zhì)水平及氧化應(yīng)激狀態(tài)。結(jié)果治療后觀察組患者C反應(yīng)蛋白(CRP),腫瘤壞死因子(TNF)-α及白細(xì)胞介素(IL)-6水平均顯著低于對(duì)照組(P0.05),丙二醛(MDA)、晚期蛋白氧化產(chǎn)物(AOPPs)水平均顯著低于對(duì)照組(P0.05),超氧化物歧化酶(SOD)水平顯著高于對(duì)照組(P0.05)。治療后觀察組患者總膽固醇(TC)、低密度脂蛋白(LDL)、甘油三酯(TG)、糖化血紅蛋白(Hb A1c)和高密度脂蛋白(HDL)水平均顯著低于對(duì)照組(P0.05);餐后2 h血糖、尿素氮(BUN)及尿蛋白水平均顯著低于對(duì)照組(P0.05)。結(jié)論血液灌流聯(lián)合序貫透析能夠有效清除老年終末期DN患者炎癥介質(zhì),緩解機(jī)體氧化應(yīng)激狀態(tài),從而有助于控制血糖水平,改善腎功能,提高臨床療效。
[Abstract]:Objective to study the effects of hemoperfusion combined with sequential dialysis on inflammatory mediators and oxidative stress in elderly patients with end-stage diabetic nephropathy.Methods 92 patients with end-stage DN were divided into control group and observation group according to different treatment methods.The observation group was treated with hemoperfusion and sequential dialysis twice a week, while the control group was treated with hemodialysis three times a week.The levels of blood glucose, renal function, inflammatory mediators and oxidative stress were evaluated in both groups.Results after treatment, the levels of C-reactive protein (CRP), tumor necrosis factor TNF- 偽 (TNF- 偽) and interleukin-6 (IL-6) in the observation group were significantly lower than those in the control group (P 0.05), and the levels of malondialdehyde (MDA) MDAA, advanced protein oxidation product (AOPPs) were significantly lower than those in the control group (P 0.05), and the levels of superoxide disambiguation were significantly lower than those in the control group.The level of SOD was significantly higher than that of control group (P 0.05).After treatment, the levels of total cholesterol, low density lipoprotein (LDLN), triglyceride (TGN), HbA1c (glycosylated hemoglobin) and high density lipoprotein (HDL) in the observation group were significantly lower than those in the control group (P 0.05), and the blood glucose at 2 hours postprandial was significantly lower in the observation group than in the control group.The levels of urea nitrogen bun and urine protein were significantly lower than those of the control group (P 0.05).Conclusion Hemoperfusion combined with sequential dialysis can effectively remove inflammatory mediators and relieve oxidative stress in elderly patients with end-stage DN, which can help to control blood glucose level, improve renal function and improve clinical efficacy.
