【摘要】：目的 通過建立雄性大鼠膀胱出口部分梗阻模型，觀察雄性大鼠膀胱出口部分梗阻后不同梗阻時(shí)期的膀胱逼尿肌細(xì)胞增生、凋亡及間質(zhì)中膠原纖維的變化情況。 方法 選取Wistar雄性大鼠40只，通過前期對(duì)建立大鼠膀胱出口部分梗阻模型不同方法的比較，本實(shí)驗(yàn)研究采用經(jīng)會(huì)陰途徑球部尿道部分結(jié)扎的方法建立大鼠膀胱出口部分梗阻模型，隨機(jī)分為假手術(shù)組10只，梗阻2周組15只，梗阻4周組15只。采用SABC免疫組化方法分析膀胱逼尿肌細(xì)胞增殖細(xì)胞核抗原PCNA表達(dá)情況；通過末端轉(zhuǎn)移酶標(biāo)記法（TUNEL法）檢測(cè)膀胱逼尿肌細(xì)胞凋亡指數(shù)；運(yùn)用VG染色法分析膠原纖維占膀胱逼尿肌面積比例變化情況。 結(jié)果 1、實(shí)驗(yàn)選取雄性Wistar大鼠40只，最終成活33只，，1只大鼠因麻醉過深死亡，2只大鼠因感染嚴(yán)重而死亡，4只大鼠因結(jié)扎過緊致膀胱破裂而死亡。 2、假手術(shù)組、梗阻2周組和梗阻4周組膀胱逼尿肌細(xì)胞PCNA陽性表達(dá)率分別為（17.19±1.37）%、（50.74±3.15）%、（33.58±2.80）%，經(jīng)統(tǒng)計(jì)方差分析，梗阻組之間差異有統(tǒng)計(jì)學(xué)意義（P＜0.05），梗阻組與假手術(shù)組之間差異有統(tǒng)計(jì)學(xué)意義（P＜0.05）。 3、假手術(shù)組、梗阻2周組和梗阻4周組逼尿肌細(xì)胞凋亡指數(shù)分別為（20.22±1.70）%、（33.90±1.86）%、（51.18±3.30）%，經(jīng)統(tǒng)計(jì)方差分析，梗阻組之間差異有統(tǒng)計(jì)學(xué)意義（P＜0.05），梗阻組與假手術(shù)組之間差異有統(tǒng)計(jì)學(xué)意義（P＜0.05）。 4、假手術(shù)組、梗阻2周組和梗阻4周組膀胱間質(zhì)中膠原纖維占膀胱逼尿肌面積比例分別為（7.07±0.67）%、（13.19±1.50）%、（22.98±2.89）%，經(jīng)統(tǒng)計(jì)方差分析，梗阻組之間差異有統(tǒng)計(jì)學(xué)意義（P＜0.01），梗阻組與假手術(shù)組之間差異有統(tǒng)計(jì)學(xué)意義（P＜0.01）。 結(jié)論 不同梗阻時(shí)期大鼠膀胱出口部分梗阻的膀胱逼尿肌形態(tài)結(jié)構(gòu)的變化，可能是膀胱逼尿肌細(xì)胞增生、凋亡以及間質(zhì)中膠原纖維占膀胱逼尿肌比例變化的綜合作用結(jié)果。
[Abstract]:PurposeBy establishing a male rat model of partial bladder outlet obstruction, the proliferation, apoptosis and collagen fibers in interstitial tissue of bladder detrusor cells in different obstructive stages after partial bladder outlet obstruction in male rats were observed.MethodForty male Wistar rats were selected to establish the model of bladder outlet partial obstruction by comparing different methods in the early stage. The method of partial urethral ligation via perineal approach was used to establish the model of bladder outlet partial obstruction in rats.They were randomly divided into sham operation group (n = 10), obstruction group (n = 15) and obstruction group (n = 15).The expression of proliferating cell nuclear antigen (PCNA) in bladder detrusor cells was analyzed by SABC immunohistochemical method, and apoptosis index of bladder detrusor cells was detected by terminal transferase labeling method (Tunel).The ratio of collagen fibers to the area of bladder detrusor muscle was analyzed by VG staining.Result1. Forty male Wistar rats were selected and 33 rats survived. 2 rats died because of severe infection and 4 rats died of bladder rupture caused by too tight ligation.2. The positive expression rate of PCNA in bladder detrusor muscle cells in sham operation group, obstruction 2 week group and obstruction 4 week group was 17.19 鹵1.37 and 50.74 鹵3.15 respectively. The difference between obstruction group and sham operation group was statistically significant (P < 0.05), and there was significant difference between obstruction group and sham operation group (P < 0.05).3The apoptotic index of detrusor cells in sham operation group, obstruction 2 week group and obstruction 4 week group were 20.22 鹵1.70 and 33.90 鹵1.86 respectively. The difference was statistically significant (P < 0.05) between obstruction group and sham operation group (P < 0.05).(4) the proportion of collagen fibers in bladder stroma in sham operation group, obstruction 2 week group and obstruction 4 week group were 7.07 鹵0.67 and 13.19 鹵1.50 respectively.There was significant difference between obstruction group and sham operation group (P < 0.01).ConclusionThe morphologic changes of bladder detrusor in rats with partial bladder outlet obstruction at different stages of obstruction may be the result of the proliferation of bladder detrusor cells apoptosis and the changes of collagen fibers in bladder detrusor muscle.
