【摘要】：目的:報告1例罕見的附睪脂肪肉瘤伴腹膜后及腹腔內多次轉移病例,提高對本病的診治水平。方法:回顧分析1例附睪脂肪肉瘤伴腹膜后及腹腔內多次轉移患者的臨床資料,結合國內外相關文獻,探討附睪脂肪肉瘤的臨床表現、診斷及治療。結果:經腹股溝切口行根治性高位睪丸切除術完整切除腫瘤,10年后出現腹膜后復發(fā)轉移,腫塊上至腎臟上極,下達盆腔,大小15.6 cm×9.4 cm×25.5 cm,包裹左腎及輸尿管上段,行左腎及輸尿管全長、腹膜后腫瘤切除手術,4年后再次出現腹膜后、腹腔內多發(fā)復發(fā)轉移灶,再次行手術切除,隨訪1年,死于肺功能衰竭。結論:附睪脂肪肉瘤極為罕見,仔細的臨床體檢和影像學檢查可協助診斷,但最終需病理確診,應早期行根治性手術切除。放化療對附睪脂肪肉瘤的作用存在爭議。本病有較高的復發(fā)率,長期的密切隨訪是必要的。
[Abstract]:Objective: to report a rare case of epididymal liposarcoma with retroperitoneal and intraperitoneal metastasis in order to improve the diagnosis and treatment of the disease.Methods: the clinical data of a case of epididymal liposarcoma with retroperitoneal and intraperitoneal metastases were retrospectively analyzed and the clinical manifestations, diagnosis and treatment of epididymal liposarcoma were discussed.Results: radical high orchiectomy was performed through inguinal incision. 10 years later, retroperitoneal recurrence and metastasis occurred. The tumor reached the upper pole of the kidney and reached the pelvic cavity. The size of the tumor was 15.6 cm 脳 9.4 cm 脳 25.5 cm, which wrapped the left kidney and the upper ureter.The whole length of left kidney and ureter was performed, the retroperitoneal tumor was resected. 4 years later, there were multiple recurrence and metastasis foci in the abdominal cavity. The patients were followed up for 1 year and died of pulmonary failure.Conclusion: epididymal liposarcoma is very rare. Careful clinical examination and imaging examination can be helpful in diagnosis, but it is necessary to make the final diagnosis by pathology, and radical resection should be carried out early.The effect of radiotherapy and chemotherapy on epididymal liposarcoma is controversial.This disease has a high recurrence rate, long-term close follow-up is necessary.
