【摘要】：目的1、探討前列腺穿刺活檢患者的血糖對前列腺癌檢出率和Gleason評分的影響。2、探討前列腺穿刺活檢中前列腺癌患者的血糖與Gleason評分、前列腺特異性抗原（tPSA）、體重指數(shù)（BMI）、前列腺總體積(TPV)和年齡等因素之間的相互關(guān)系。 方法前瞻性收集2010年2月～2013年4月在蘇州大學(xué)附屬第一醫(yī)院初次行經(jīng)直腸前列腺穿刺活檢患者的血糖、Gleason評分、tPSA、BMI、TPV、年齡等臨床資料，所有患者以血糖6.1mmol/L為界分成兩組，采用2檢驗比較高血糖組和正常血糖組前列腺癌檢出率和Gleason評分的差異，，運用Spearman等級相關(guān)分析血糖與Gleason評分、tPSA、BMI、TPV、年齡等之間的相互關(guān)系。 結(jié)果452例前列腺穿刺活檢患者中，前列腺癌183例，高血糖組和正常血糖組前列腺癌檢出率分別為43.00%和39.77%，差異沒有統(tǒng)計學(xué)顯著意義；高血糖組前列腺癌患者中低Gleason評分者（Gleason評分＜7分）占20.93%，明顯高于正常血糖組前列腺癌的6.43%（P＜0.05），高血糖組前列腺癌患者中低Gleason評分的優(yōu)勢比（OR）為3.85（95%CI：1.42-10.40），高Gleason評分（Gleason評分≥7分）的OR為0.26（95%CI：0.10-0.70）。Spearman等級相關(guān)分析顯示前列腺癌患者的血糖值與Gleason評分呈負(fù)相關(guān)(r=-0.256，P＜0.05)，與BMI呈正相關(guān)(r=0.267，P＜0.05)，與年齡、TPV、tPSA和I-PSS評分相關(guān)性無統(tǒng)計學(xué)意義(P＞0.05)。 結(jié)論前列腺穿刺活檢患者的血糖對前列腺癌檢出率沒有明顯影響。高血糖組前列腺癌患者中低Gleason評分的OR明顯高于高Gleason評分的OR，血糖是影響前列腺穿刺活檢患者前列腺癌Gleason評分的一個獨立因素。
[Abstract]:Objective : To investigate the effects of blood glucose on the detection rate of prostate cancer and Gleason score in patients with prostate biopsy , and investigate the correlation between the blood glucose and Gleason score , prostate specific antigen ( tPSA ) , body mass index ( BMI ) , total prostate volume ( TPV ) and age in prostate biopsy .

Methods The blood glucose , Gleason score , tPSA , BMI , TPV and age were prospectively collected from February 2010 to April 2013 at the First Affiliated Hospital of Suzhou University . All patients were divided into two groups with blood glucose of 6.1 mmol / L . The correlation between blood glucose and Gleason score , tPSA , BMI , TPV , age and the like was analyzed .

Results Among the 452 cases of prostate biopsy , 183 cases of prostate cancer , 43 . 00 % and 39.77 % of normal blood sugar group were detected , and the difference was not statistically significant .
