【摘要】：目的:檢測(cè)Ki-67在T1期非肌層浸潤(rùn)性膀胱癌(non-muscle invasive bladder cancer,NMIBC)組織中的表達(dá),并探討其與腫瘤復(fù)發(fā)和進(jìn)展的關(guān)系。方法:回顧性分析2011年6月至2013年10月天津醫(yī)科大學(xué)腫瘤醫(yī)院102例T1期NMIBC患者的臨床病理資料,利用免疫組織化學(xué)方法檢測(cè)組織中Ki-67的表達(dá),分析Ki-67表達(dá)與患者臨床病理特征的關(guān)系,探討其對(duì)T1期NMIBC復(fù)發(fā)和進(jìn)展的影響。結(jié)果:中位隨訪時(shí)間43(24~57)個(gè)月,102例T1期NMIBC患者中20例(19.6%)復(fù)發(fā),12例(11.8%)進(jìn)展,32例(31.4%)Ki-67表達(dá)≥25%。Ki-67表達(dá)與腫瘤分級(jí)相關(guān)(P0.05),與患者性別、年齡、腫瘤數(shù)目、腫瘤大小等無(wú)相關(guān)性(P0.05)。單因素分析結(jié)果顯示,Ki-67表達(dá)與T1期NMIBC的復(fù)發(fā)無(wú)相關(guān)性(P0.05),Ki-67表達(dá)、腫瘤分級(jí)、腫瘤數(shù)目和既往復(fù)發(fā)率是影響T1期NMIBC進(jìn)展的危險(xiǎn)因素(P0.05),Cox風(fēng)險(xiǎn)回歸模型多因素分析結(jié)果顯示,Ki-67高表達(dá)(P=0.043)和既往復(fù)發(fā)率(P=0.018)是影響T1期NMIBC進(jìn)展的獨(dú)立危險(xiǎn)因素。結(jié)論:Ki-67表達(dá)是T1期NMIBC的獨(dú)立預(yù)后因素,檢測(cè)Ki-67表達(dá)有助于預(yù)測(cè)其進(jìn)展風(fēng)險(xiǎn),為采取及時(shí)有效治療提供依據(jù)。
