體外沖擊波碎石成功率密度分布的能量效應(yīng)

【摘要】:體外沖擊波碎石(ESWL)是一種無創(chuàng)治療泌尿系結(jié)石的醫(yī)療技術(shù),臨床上有95%以上的成功率,沖擊波能量是這種醫(yī)療技術(shù)成功的關(guān)鍵因素。為此,闡述了沖擊波的形成、傳播、聚焦,沖擊波發(fā)生器的電場(chǎng)能量和碎石原理等,然后從ESWL成功率密度分布中分析沖擊波最佳能量。ESWL病例入選條件是門診輸尿管結(jié)石患者,年齡在18~83歲之間,左、右單側(cè)或雙側(cè)輸尿管具有放射影像學(xué)結(jié)石特征,ESWL數(shù)據(jù)完整,并且輸尿管結(jié)石ESWL成功者。通過建立能量的ESWL成功率密度分布函數(shù),得到其數(shù)學(xué)期望和均方差,從而獲得ESWL成功的最佳能量。研究結(jié)果表明:從入選的5 747例ESWL病例中,獲得ESWL成功率密度分布函數(shù)的數(shù)學(xué)期望為535 m J/mm~2,均方差為157 m J/mm~2,最佳能量在418~732 m J/mm~2范圍內(nèi);該最佳能量可以通過能級(jí)來確定,能級(jí)一般分為Ⅰ級(jí)(約10 m J/mm~2)、Ⅱ級(jí)(20~40 m J/mm~2)、Ⅲ級(jí)(30~300 m J/mm~2)、Ⅳ級(jí)(400~1 200 m J/mm~2);可以根據(jù)結(jié)石的大小和密度來選擇能級(jí)。該結(jié)果有助于臨床醫(yī)師根據(jù)實(shí)際情況選擇能量,以提高ESWL成功率。
[Abstract]:Extracorporeal shock wave lithotripsy (ESWL) is a non-invasive medical technique for the treatment of urolithiasis with a clinical success rate of more than 95%. Shock wave energy is the key factor for the success of this medical technique.Therefore, the formation, propagation, focus, electric field energy of shock wave generator and the principle of lithotripsy were expounded. The optimal energy of shock wave was analyzed from the density distribution of ESWL success rate. The selected condition of the case was the outpatient patients with ureterolithiasis.The left, right and unilateral ureters had radiographic calculi. The data of ESWL were complete and the ureteral calculi were successful in ESWL.By establishing the density distribution function of ESWL success rate of energy, the mathematical expectation and mean square deviation are obtained, and the optimum energy of ESWL success is obtained.The results showed that the mathematical expectation of the density distribution function of ESWL success rate was 535mJ / mm2, the mean square deviation was 157mJ / mm2, and the optimum energy was within 418732m J/mm~2, which could be determined by the energy level.The energy levels are generally divided into 鈪,
