【摘要】：目的探討腹腔鏡Sandwich法尿道及周圍組織重建技術(shù)對高風(fēng)險組前列腺癌根治術(shù)后早期尿控的療效。方法納入2011年1月至2015年9月于本院治療的高風(fēng)險前列腺癌患者71例,采用腹腔鏡Sandwich法尿道及周圍組織重建技術(shù)47例,采用單純尿道吻合技術(shù)24例,對比兩組患者年齡、體質(zhì)量指數(shù)、前列腺體積、術(shù)前前列腺特異性抗原(prostate specific antigen,PSA)、臨床分期、手術(shù)時間、術(shù)后引流管拔除時間、術(shù)后尿管拔除時間、病理分期、Gleason評分、術(shù)后切緣陽性率、淋巴結(jié)陽性率及拔除尿管后第2、4、12、24、52周尿控恢復(fù)情況。結(jié)果兩組患者間年齡、體質(zhì)量指數(shù)、前列腺體積、術(shù)前PSA、臨床分期、病理分期、Gleason評分、術(shù)后切緣陽性率、淋巴結(jié)陽性率、術(shù)后引流管拔除時間的差異均無統(tǒng)計學(xué)意義(P0.05);Sandwich法尿道及周圍組織重建組手術(shù)時間長于對照組(P=0.024)、術(shù)后尿管拔除時間顯著早于對照組(P0.001),拔除尿管第12周尿控恢復(fù)顯著優(yōu)于對照組(P=0.023)。結(jié)論腹腔鏡Sandwich法尿道及周圍組織重建技術(shù)安全可行,顯著改善高風(fēng)險組前列腺癌根治術(shù)后早期尿控。
[Abstract]:Objective to investigate the effect of laparoscopic Sandwich urethral and peripheral tissue reconstruction on early urinary control after radical resection of prostate cancer in high risk group. Methods 71 patients with high risk prostate cancer who were treated in our hospital from January 2011 to September 2015 were included in this study. Laparoscopic Sandwich technique was used in 47 cases of urethral and peripheral tissue reconstruction, and simple urethral anastomosis was performed in 24 cases. The age, body mass index, prostate volume, specific antigen-PSAs and clinical stages were compared between the two groups. Time of operation, time of extubation of drainage tube, time of extubation, Gleason score of pathological stage, positive rate of incision margin, positive rate of lymph node and recovery of urinary control at 2 ~ (th) ~ (th) ~ (th) ~ (th) ~ (th) ~ (th) ~ (24) ~ (th) week after extubation of urethral catheter. Body mass index (BMI), prostate volume, preoperative PSAs, clinical stage, pathological stage and Gleason score, positive rate of incision margin, positive rate of lymph node, There was no significant difference in the time of extubation of drainage tube after operation. The operation time of urethral and surrounding tissue reconstruction group was longer than that of control group. The time of extubation of urinary tube was significantly earlier than that of control group (P 0.001), and the urinary control recovered significantly at the 12th week after extubation. Conclusion Laparoscopic Sandwich method is safe and feasible for urethral and peripheral tissue reconstruction. Significantly improved early urinary control after radical prostatectomy in high risk groups.
