【摘要】：目的:回顧性分析手術(shù)去勢與藥物去勢對晚期前列腺癌患者的生活質(zhì)量的影響。方法:選取2014年1月—2016年3月在我院行去勢治療的125例患者作為研究對象,按其治療方法的不同分為手術(shù)去勢治療組和藥物去勢治療組,其中手術(shù)治療組59例,藥物治療組66例。應(yīng)用癌癥患者生命質(zhì)量測定量表(cancer quality of life questionnaire-30,QLQ-C30)及前列腺癌患者生活質(zhì)量子量表(quality of life questionnaire of prostate cancer,QLQ-PR25)分別對治療前、治療后6個月、治療后1年的患者生活質(zhì)量進行評價。結(jié)果:與治療前相比較,手術(shù)去勢組與藥物去勢組患者總體健康狀況均明顯提高,排尿癥狀得到改善,疼痛癥狀得以緩解。手術(shù)去勢組術(shù)后經(jīng)濟負擔減輕,患者軀體功能、角色功能、社會功能、情緒功能均有所下降,疲乏癥狀加重,治療相關(guān)癥狀如乳房脹痛、面色潮紅等癥狀出現(xiàn),性趣顯著下降,性生活喪失。藥物去勢組在治療后6個月患者經(jīng)濟負擔加重,軀體功能、角色功能、社會功能、情緒功能有所下降,但在治療1年后,患者經(jīng)濟負擔減輕,角色、社會、情緒功能有所恢復(fù),在治療6個月后,患者出現(xiàn)的治療相關(guān)癥狀,如乳房脹痛、面色潮紅等明顯加重,在治療1年后可明顯減輕。性趣、性生活在治療6個月后明顯降低甚至喪失,但在治療1年后有一定程度的恢復(fù)。結(jié)論:兩種去勢方式均可對前列腺癌患者產(chǎn)生不同程度的影響,手術(shù)去勢對患者的生活質(zhì)量的影響是不可逆的,尤其是對性功能的影響,而藥物去勢對患者的部分生活質(zhì)量的影響是可逆的。
[Abstract]:Objective: to retrospectively analyze the effect of surgical castration and drug castration on the quality of life of patients with advanced prostate cancer. Methods: 125 patients who were treated in our hospital from January 2014 to March 2016 were selected as study subjects. According to the different treatment methods, the patients were divided into two groups: the surgical castration group and the drug castration group, among which 59 cases were treated by the surgical treatment group. 66 patients in the drug treatment group were treated with quality of life questionnaire-30 QLQ-C30 and quality of life questionnaire of prostate cancer-QLQ-PR25 before and 6 months after treatment, respectively. Results: compared with those before treatment, the overall health status of patients in the castration group and the drug castration group were significantly improved, and the urination symptoms were improved. The pain symptoms were alleviated. In the castration group, the economic burden was reduced, the patients' somatic function, role function, social function, emotional function were decreased, the symptoms of fatigue were aggravated, and the treatment of related symptoms such as breast pain, Symptoms such as complexion flashes, sexual interest decreased significantly, sexual life was lost. Patients in the castration group had increased financial burden, body function, role function, social function and emotional function 6 months after treatment, but one year after treatment, the patients in the drug castration group had decreased financial burden, body function, role function, social function and emotional function, but after one year of treatment, After 6 months of treatment, the patients' symptoms, such as breast distension and facial redness, were obviously aggravated, and after one year's treatment, the patients' sexual interest could be significantly alleviated, and the patients' economic burden, role, social and emotional functions were somewhat improved, and after 6 months of treatment, the patients' symptoms, such as breast distension and flushing, were obviously aggravated. Sexual life was significantly decreased or even lost after 6 months of treatment, but recovered to a certain extent after one year of treatment. Conclusion: both castration methods have different effects on prostate cancer patients. The effect of surgical castration on the quality of life of patients is irreversible, especially on sexual function, while the effect of drug castration on part of quality of life is reversible.
