【摘要】：目的探討血清同型半胱氨酸(Hcy)、胱抑素C(Cys C)聯(lián)合尿足細(xì)胞檢測(cè)對(duì)2型糖尿病腎病(DN)的早期診斷價(jià)值。方法收集130例臨床診斷為2型糖尿病患者,依據(jù)尿白蛋白/肌酐比值(UACR)將其分為正常白蛋白尿(NA)組42例、微量白蛋白尿(MA)組45例和大量白蛋白尿(LA)組43例;選取同期健康體檢者40例作為對(duì)照組。應(yīng)用全自動(dòng)生化分析儀檢測(cè)血清Hcy和Cys C水平,采用流式細(xì)胞術(shù)檢測(cè)尿中足細(xì)胞數(shù)量;分析血清Hcy、Cys C、尿足細(xì)胞與UACR的關(guān)系,并應(yīng)用ROC曲線確定單一和聯(lián)合檢測(cè)診斷早期DN的價(jià)值。結(jié)果 NA、MA、LA組血清Hcy、Cys C水平及尿足細(xì)胞數(shù)量均高于對(duì)照組,并隨著DN進(jìn)展而升高(P均0.05)。相關(guān)分析結(jié)果表明,血清Hcy、Cys C水平及尿足細(xì)胞數(shù)量分別與UACR呈正相關(guān)(r=0.83、0.83、0.84,P均0.05)。在NA組中,Hcy、Cys C和足細(xì)胞診斷早期DN的敏感性分別為59.5%、80.9%和64.3%,三者聯(lián)合檢測(cè)的敏感性為90.8%。結(jié)論 2型糖尿病患者腎損傷時(shí)血清Hcy、Cys C水平和尿足細(xì)胞數(shù)量顯著升高,三者聯(lián)合檢測(cè)對(duì)于DN早期診斷具有重要價(jià)值。
[Abstract]:Objective to investigate the value of serum homocysteine cystatin, cystatin Cys Cys and urinary podocyte in the early diagnosis of type 2 diabetic nephropathy. Methods 130 patients with type 2 diabetes mellitus were collected. According to urinary albumin / creatinine ratio UACRs were divided into normal albuminuria (NAA) group (n = 42), microalbuminuria (MAA) group (n = 45) and large albuminuria group (n = 43). Serum Hcy and Cys C levels were measured by automatic biochemical analyzer, the number of podocytes in urine was detected by flow cytometry, and the relationship between urinary podocytes and UACR was analyzed. ROC curve was used to determine the value of single and combined detection in the diagnosis of early DN. Results the levels of serum Hcytys C and the number of urinary podocytes in NAA MAGLA group were higher than those in the control group, and increased with the progression of DN (P < 0.05). There was a positive correlation between the level of serum Hcytys C and the number of urinary podocytes and UACR. In na group, the sensitivity of HcyCys C and podocyte in diagnosing early DN were 59.5% and 64.3%, respectively. The sensitivity of the combined detection was 90.8%. Conclusion Type 2 diabetes mellitus has a sensitivity of 90.80.Conclusion Type 2 diabetes mellitus has a sensitivity of 90.8%. During renal injury, the levels of serum HcyCys C and the number of urinary podocytes were significantly increased. The combined detection has important value for early diagnosis of DN.
