【摘要】：目的:探討采用精囊鏡治療以血精為表現(xiàn)的精囊炎的方法。方法:2012年4月至2014年12月,64例精囊炎患者均行精囊鏡治療,術(shù)中抽取精囊液行細(xì)菌和UU、CT、Mh培養(yǎng),并清除精囊結(jié)石、炎癥物質(zhì),精囊內(nèi)保留灌注左旋氧氟沙星注射液0.3 g/100 ml。術(shù)后定期復(fù)查。結(jié)果:64例患者順利完成手術(shù),手術(shù)時(shí)間25~50(40±15)min。術(shù)中于后尿道精阜表面尋及射精管開(kāi)口者2例,前列腺小囊內(nèi)見(jiàn)異常通道者8例,經(jīng)前列腺小囊內(nèi)破窗者54例,其中32例采用精囊鏡直接破壁,22例采用鈥激光前列腺小囊內(nèi)破壁。術(shù)中發(fā)現(xiàn)前列腺小囊內(nèi)結(jié)石5例,精囊結(jié)石6例,精囊與精阜小囊均有結(jié)石者2例,精囊液細(xì)菌培養(yǎng)陽(yáng)性1例,為洛菲不動(dòng)桿菌,UU、CT、Mh均為陰性。術(shù)后個(gè)3個(gè)月血精完全消失52例,血精變淡者8例,血精無(wú)變化者4例。結(jié)論:精囊鏡經(jīng)前列腺小囊內(nèi)開(kāi)窗引流術(shù)手術(shù)時(shí)間短,療效確切,且能對(duì)精囊進(jìn)行檢查、清除結(jié)石和炎癥物質(zhì),無(wú)明顯并發(fā)癥,值得臨床推廣。
[Abstract]:Objective: to explore the treatment of seminal vesiculitis with seminal vesicle by seminal vesicle. Methods: from April 2012 to December 2014, 64 cases of seminal vesiculitis were treated with seminal vesicle. Inflammatory substances were preserved and perfused into the seminal vesicle with levofloxacin injection 0.3 g / 100 ml. The results showed that 64 cases of levoofloxacin were successfully operated after operation, the operative time was 2550 ~ 40 鹵15 min. 2 cases were located and ejaculatory duct orifice on the surface of posterior urethral sputum during the operation. Abnormal channels were found in 8 cases of prostatic microsac, 54 cases were broken through prostatic microsac window, 32 cases were directly broken wall by seminal vesicle in 22 cases, 5 cases were found intravesicular stones in prostatic microsac during operation, 22 cases were directly broken by seminal vesicle, 22 cases were treated with holmium laser. Seminal vesicle calculi were found in 6 cases, seminal vesicle and caruncle vesicle in 2 cases, seminal vesicle fluid culture positive in 1 case, acinetobacter UUU CTMh negative, blood spermatozoa disappeared completely in 52 cases 3 months after operation, blood essence became light in 8 cases. Conclusion: the operation time of seminal vesicle is short, the curative effect is accurate, the seminal vesicle can be checked, the stone and inflammatory substance can be cleared, and there is no obvious complication, so it is worth popularizing clinically.
