【摘要】：目的探討前列腺健康指數(shù)(PHI)對(duì)血清PSA在4～10ug/L、直腸指診(-)患者前列腺癌的診斷價(jià)值。方法選擇101例血清前列腺特異性抗原(PSA)在4-10ug/L、直腸指診(-)并接受經(jīng)直腸B超引導(dǎo)下前列腺穿刺活檢的老年男性的臨床資料,其中前列腺癌16例、良性病變85例。檢測(cè)血清tPSA、fPSA和p2PSA,分別計(jì)算%fPSA、 %p2PSA和PHI,以前列腺穿刺活檢作為前列腺癌確診依據(jù),建立tPSA、%fPSA、 p2PSA、%p2PSA和PHI的受試者工作特征曲線,計(jì)算曲線下面積,分析tPSA、%fPSA、p2PSA、%p2PSA和PHI診斷前列腺癌的價(jià)值以及PHI預(yù)判前列腺癌侵襲性的價(jià)值。結(jié)果前列腺癌組的tPSA、fPSA、p2PSA、%fPSA、%p2PSA和PHI均值分別為6.94ug/L、1.27 ug/L、21.49 ng/L、18.71%、1.59%、41.51,區(qū)別于非前列腺癌組的6.68ug/L、1.34 ug/L、13.37 ng/L、20.42%、1.05%、27.18; PHI、%p2PSA、 p2PSA、%fPSA和tPSA的診斷前列腺癌的曲線下面積分別為0.777、0.769、0.681、0.590、0.580。前列腺癌組中Gleason評(píng)分≥7組PHI均值明顯大于Gleason評(píng)分≤6組PHI均值,其差異具有統(tǒng)計(jì)學(xué)意義。結(jié)論P(yáng)HI能進(jìn)一步提高直腸指診(-)、血清PSA在4～10ug/L患者的前列腺癌診斷準(zhǔn)確性,具有比%p2PSA、p2PSA、%fPSA、tPSA更高的診斷價(jià)值,而且PHI值與前列腺癌侵襲性呈正相關(guān)。
[Abstract]:Objective to investigate the diagnostic value of prostate health index (PHI) for prostate cancer in patients with serum PSA at 4 ~ 10ugr / L, rectal finger diagnostics. Methods 101 patients with serum prostate specific antigen (PSA) were selected at 4-10ugr-L) and received transrectal ultrasound guidance.
