【摘要】：目的：通過檢測IDO、FoxP3在兒童腎母細(xì)胞瘤組織中的表達(dá)，分析其與腎母細(xì)胞瘤臨床病理學(xué)參數(shù)的相關(guān)性，研究IDO、FoxP3在腎母細(xì)胞瘤免疫耐受過程中的作用，并探討二者能否作為判斷兒童腎母細(xì)胞瘤預(yù)后的指標(biāo)。 方法：回顧性分析重慶醫(yī)科大學(xué)附屬兒童醫(yī)院泌尿外科2010.7-2011.8期間診斷為腎母細(xì)胞瘤的臨床資料，，包括患兒性別、年齡、臨床分期、病例類型，應(yīng)用免疫組織化學(xué)法對IDO和FoxP3在兒童腎母細(xì)胞瘤中的表達(dá)進(jìn)行定位和半定量分析，并將其與患兒性別、發(fā)病年齡、腫瘤臨床分期、病理類型進(jìn)行相關(guān)性分析。 結(jié)果：20例腎母細(xì)胞瘤中有10例IDO呈陽性表達(dá)，陽性表達(dá)率為50%。6例FoxP3陽性表達(dá)，陽性表達(dá)率為30%。IDO和FoxP3在同一樣本中均陽性表達(dá)的有5例，陽性率為25%。20例中有9例IDO和FoxP3均為陰性。同一樣本中IDO陽性和FoxP3陰性者5例（25%），同一樣本中IDO陰性和FoxP3陽性者1例（20%）; IDO、FoxP3的表達(dá)與腎母細(xì)胞瘤發(fā)病年齡、患兒性別、腫瘤臨床分期、病理類型均無顯著相關(guān)性（P＞0.05）；IDO和FoxP3二者的表達(dá)具有正相關(guān)性（P＜0.05）；同時表達(dá)IDO和FoxP3的腫瘤在患兒性別、年齡、腫瘤臨床分期、病理類型等方面均無顯著差異（P＞0.05）。20例腎母細(xì)胞瘤中1例復(fù)發(fā)，但其IDO、FoxP3表達(dá)均為陰性。 結(jié)論：IDO和FoxP3在兒童腎母細(xì)胞瘤中陽性率低，提示二者可能不參與腎母細(xì)胞瘤免疫耐受的形成，其原因可能是腎母細(xì)胞瘤作為一種胚胎性腫瘤，與成人腫瘤發(fā)病機(jī)制不同。IDO的表達(dá)與FoxP3的表達(dá)呈正相關(guān)，提示在腎母細(xì)胞瘤中IDO和FoxP3+Tregs之間可能存在相互作用。因本研究樣本量有限，IDO和FoxP3在腎母細(xì)胞瘤免疫耐受中的作用尚需進(jìn)一步驗證，其能否作為判斷兒童腎母細(xì)胞瘤預(yù)后的臨床指標(biāo)還有待深入研究。
[Abstract]:Objective: to study the role of IDO- FoxP3 in immunological tolerance of nephroblastoma by detecting the expression of IDO- FoxP3 in children's nephroblastoma, and analyzing its correlation with clinicopathological parameters of nephroblastoma. To explore whether they can be used as a prognostic index for children with nephroblastoma. Methods: the clinical data of nephroblastoma diagnosed in Urology Department, affiliated Children's Hospital of Chongqing Medical University, were retrospectively analyzed, including sex, age, clinical stage and type of cases. The expression of IDO and FoxP3 in children's nephroblastoma was determined by immunohistochemical method. The expression of IDO and FoxP3 was correlated with sex, age, clinical stage and pathological type of children. Results among 20 cases of nephroblastoma, 10 cases were positive for IDO, and the positive expression rate was 50.6 cases. The positive expression rate of 30. IDO and FoxP3 in the same sample was 5 cases. The positive rate of IDO and FoxP3 was negative in 9 out of 25. 20 cases. In the same sample, 5 cases were IDO positive and FoxP3 negative, 1 case was IDO negative and 1 case was FoxP3 positive in the same sample, and the expression of IDOX FoxP3 was associated with the onset age, sex, clinical stage of tumor. There was no significant correlation between the expression of IDO and FoxP3 in pathological types (P > 0.05) and the positive correlation between the expression of IDO and FoxP3 (P < 0.05), and the expression of IDO and FoxP3 in children with sex, age, clinical stage of tumor. There was no significant difference in pathological types (P > 0.05). One of 20 cases of nephroblastoma recurred, but the expression of FoxP3 was negative. Conclusion the low positive rate of FoxP3 and VIDO in children nephroblastoma suggests that they may not participate in the formation of immune tolerance in nephroblastoma, which may be due to the fact that Wilms tumor is an embryonic tumor. The expression of. IDO was positively correlated with the expression of FoxP3, which was different from the pathogenesis of adult tumors. It is suggested that there may be interaction between IDO and FoxP3 Tregs in nephroblastoma. The role of IDO and FoxP3 in immunological tolerance of nephroblastoma needs to be further verified. Whether it can be used as a clinical index to judge the prognosis of child nephroblastoma remains to be further studied.
