阻塞性睡眠呼吸暫停綜合征與腎損傷相關(guān)性的META分析

【摘要】:目的:阻塞性睡眠呼吸暫停綜合征可引起多系統(tǒng)器官并發(fā)癥,近年來,阻塞性睡眠呼吸暫停綜合征造成的腎臟損害引起較多的關(guān)注。血清胱抑素C、尿N-乙酰-β-D氨基葡萄糖苷、尿γ-谷氨酰轉(zhuǎn)移酶、尿素氮、肌酐常用來作為評價腎臟損傷的標志物,本研究通過評價阻塞性睡眠呼吸暫停綜合征與上述因子的相關(guān)性來反映腎臟的損害。 方法:計算機檢索PubMed、EMbase、Web of science、Cochrane libray、 CNKI、CBM、VIP、Wanfang Data電子數(shù)據(jù)庫,并手工檢索灰色文獻及納入文獻的參考文獻,檢索時間截至2013年12月,全面收集OSAHS與腎損傷相關(guān)的隊列研究和病例對照研究。由2位評價員獨立進行質(zhì)量評價、數(shù)據(jù)提取并交叉核對,如有分歧,參考第三方意見。采用RevMan5.2軟件進行Meta分析。 結(jié)果:共納入6個病例對照研究,包括659例研究對象。Meta分析結(jié)果顯示:輕、中、重度阻塞性睡眠呼吸暫停綜合征患者血清胱抑素C、尿N-乙酰-β-D氨基葡萄糖苷均高于對照組,差異具有統(tǒng)計學意義;中、重度阻塞性睡眠呼吸暫停綜合征患者血清肌酐、尿γ-谷氨酰轉(zhuǎn)移酶均高于對照組,差異具有統(tǒng)計學意義;重度阻塞性睡眠呼吸暫停綜合征患者血清尿素氮高于對照組,差異具有統(tǒng)計學意義。 結(jié)論:當前證據(jù)表明,阻塞性睡眠呼吸暫停綜合征患者存在腎損傷,在血清胱抑素C、尿N-乙酰-β-D氨基葡萄糖苷、尿γ-谷氨酰轉(zhuǎn)移酶、尿素氮、肌酐水平上較正常對照組升高,且與阻塞性睡眠呼吸暫停綜合征患者病情嚴重程度呈正相關(guān)。上述結(jié)果仍需更高質(zhì)量、更大樣本量的研究進一步證實。
[Abstract]:Objective: obstructive sleep apnea syndrome (OSAS) can cause multiple organ complications. In recent years, more attention has been paid to renal damage caused by obstructive sleep apnea syndrome (OSAS). Urinary 緯-glutamyl transferase, urea nitrogen and creatinine are often used as markers for evaluating renal injury. This study evaluated the correlation between obstructive sleep apnea syndrome (OSAS) and these factors to reflect renal damage. Methods: a computer-based search was conducted for the electronic database of PubMedus EMbase of Science Cochrane lie, CNKI CBM VIPM Wanfang Data, and the grey literature and the references included in the literature were retrieved by hand. The retrieval time was up to December 2013. The cohort study and case-control study of OSAHS related to renal injury were collected. The data were extracted and cross-checked by two evaluators. If there was disagreement, the third party's opinion was consulted. RevMan5.2 software was used for Meta analysis. Results: a total of 6 case-control studies were conducted, including 659 subjects. Meta-analysis showed that serum cystatin C and urinary N-acetyl- 尾 -D-aminoglycoside in patients with mild, moderate and severe obstructive sleep apnea syndrome were higher than those in control group. The serum creatinine and urine 緯-glutamyl transferase in patients with severe obstructive sleep apnea syndrome were significantly higher than those in the control group. The serum urea nitrogen in patients with severe obstructive sleep apnea syndrome was significantly higher than that in the control group. Conclusion: the current evidence indicates that there is renal injury in patients with obstructive sleep apnea syndrome. The levels of serum cystatin C, urine N-acetyl- 尾 -D-glucoside, urinary 緯 -glutamyl transferase, urea nitrogen and creatinine are higher than those in normal control group. There is a positive correlation with the severity of obstructive sleep apnea syndrome.
