【摘要】：目的:探討影響原發(fā)性陰道癌預(yù)后的臨床相關(guān)因素。方法:回顧性分析2003年1月至2015年1月廣西醫(yī)科大學(xué)附屬腫瘤醫(yī)院80例陰道惡性腫瘤患者臨床資料。80例患者分為放療組49例,其中二維放療(two-dimensional radiotherapy 2DRT)組29例、三維放療(three-dimensional radiotherapy 3DRT)組20例,比較兩組的療效和并發(fā)癥;手術(shù)組31例,其中腹腔鏡手術(shù)組16例、開腹手術(shù)組15例,評估腹腔鏡手術(shù)在治療原發(fā)性陰道癌中的可行性。結(jié)果:單因素生存分析結(jié)果顯示FIGO分期、病理類型、腫瘤大小、陰道受侵長度與預(yù)后相關(guān)(均P0.05),Cox多因素分析顯示FIGO分期和病理類型為影響陰道癌預(yù)后的獨立因素。2DRT組5年生存率20.90%與3DRT組的58.60%進(jìn)行比較,差異具有統(tǒng)計學(xué)意義(P=0.022);2DRT組泌尿系統(tǒng)和消化道并發(fā)癥的發(fā)生率為48.27%(14/29)和51.72%(15/29),分別與3DRT組的15.00%(3/20)和20.00%(4/20)進(jìn)行比較,差異具有統(tǒng)計學(xué)意義(均P0.05)。開腹手術(shù)組平均住院(57.00±41.75)天,多于腹腔鏡手術(shù)組的(29.56±7.3)天,兩組比較差異具有統(tǒng)計學(xué)意義(P=0.024)。結(jié)論:三維放療及腹腔鏡手術(shù)的應(yīng)用,在未降低患者生存率同時可降低并發(fā)癥,改善患者的生存質(zhì)量。
[Abstract]:Objective: to investigate the clinical related factors affecting the prognosis of primary vaginal cancer. Methods: the clinical data of 80 patients with vaginal malignant tumor in tumor Hospital affiliated to Guangxi Medical University from January 2003 to January 2015 were analyzed retrospectively. 80 patients were divided into radiotherapy group (n = 49), two-dimensional radiotherapy (two-dimensional radiotherapy 2DRT) group (n = 29) and three-dimensional radiotherapy (three-dimensional radiotherapy 3DRT) group (n = 20). The curative effects and complications of the two groups were compared. 31 cases in the operation group, including 16 cases in the laparoscopic operation group and 15 cases in the open operation group, were used to evaluate the feasibility of laparoscopic surgery in the treatment of primary vaginal cancer.
