【摘要】：目的探討陰道鏡檢查聯(lián)合宮頸人乳頭瘤病毒(HPV)檢測在宮頸病變早期診斷中的應(yīng)用價值。方法選取2010年1月-2014年12月于該院治療的212例液基細(xì)胞學(xué)檢查(LCT)呈陰性的以接觸性陰道出血為主要臨床表現(xiàn)的患者作為研究對象,回顧性分析其陰道鏡檢查及HPV檢測結(jié)果,并與宮頸活檢組織病理學(xué)進(jìn)行比較。結(jié)果 212例患者中,宮頸活檢組織病理檢查結(jié)果確診為宮頸癌者70例,確診率為33.0%。陰道鏡檢查、HPV檢測及兩者聯(lián)合與宮頸活檢病理檢查結(jié)果一致性程度分別為中度、中度和極強(K=0.567、0.473和0.832)。陰道鏡檢查聯(lián)合HPV檢測早期診斷宮頸癌的靈敏度為91.4%,明顯高于單一陰道鏡檢查和HPV檢測(均P0.05),其特異度為93.0%,與單一陰道鏡檢查和HPV檢測比較差異無統(tǒng)計學(xué)意義(均P0.05)。結(jié)論陰道鏡檢查聯(lián)合HPV檢測在早期診斷宮頸癌中具有良好的應(yīng)用價值,對于LCT檢查陰性的以接觸性陰道出血為主要臨床表現(xiàn)的患者,應(yīng)當(dāng)采用該聯(lián)合檢查方法,以提高宮頸癌的檢出率。
[Abstract]:Objective to evaluate the value of colposcopy combined with cervical human papillomavirus (HPV) in the early diagnosis of cervical lesions. Methods from January 2010 to December 2014, 212 patients with negative (LCT) and contact vaginal bleeding were selected as the subjects. The results of colposcopy and HPV were analyzed and compared with the pathological changes of cervical biopsies. Results among 212 cases, 70 cases were diagnosed as cervical cancer by pathological examination, the diagnostic rate was 33.0%.
