東營(yíng)市HPV流行和致病情況研究指導(dǎo)預(yù)防性疫苗開(kāi)發(fā)和引進(jìn)

【摘要】:目的通過(guò)檢測(cè)和統(tǒng)計(jì)東營(yíng)市HPV各亞型在健康人群和宮頸疾病患者中的流行情況,指導(dǎo)當(dāng)?shù)豀PV預(yù)防性疫苗的開(kāi)發(fā)和引進(jìn)。方法留取健康體檢婦女以及各級(jí)宮頸上皮不典型增生和宮頸癌患者的宮頸脫落細(xì)胞,采用基因?qū)Я麟s交技術(shù)檢測(cè)21種HPV亞型,統(tǒng)計(jì)HPV各亞型在健康人群和宮頸疾病患者中的流行情況,分析當(dāng)?shù)亻_(kāi)發(fā)和引進(jìn)預(yù)防性HPV疫苗時(shí)所應(yīng)重點(diǎn)選擇的亞型。結(jié)果在6 401例健康體檢人群中,HPV總感染率為15.11%(967/6 401),檢出HPV 1 145株,各HPV亞型感染所占比例排名靠前依次為16型(23.23%)、52型(16.68%)、58型(10.22%)、53型(9.34%)、33型(6.29%)、18型(5.94%);在166例HPV感染的CINⅠ~Ⅱ患者中檢出HPV亞型198株,各HPV亞型感染所占比例排名靠前依次為16型(18.37%)、58型(14.29%)、31型(14.29%)、52(10.20%)、18型(9.18%)、68型(8.16%);在31例HPV感染的≥CINⅢ患者中檢出HPV亞型38株,各HPV亞型感染所占比例排名靠前依次為16型(36.84%)、31型(18.42%)、58型(15.79%)、18型(10.52%)、68型(7.89%)、33型(5.26%)。結(jié)論因HPV亞型的流行存在地域性,且各亞型的致病能力不同,所以各地區(qū)應(yīng)綜合考慮以上兩方面因素,有針對(duì)性地開(kāi)發(fā)和引進(jìn)HPV疫苗。就東營(yíng)市而言,如果以全面降低人群HPV感染為主要目的,應(yīng)考慮開(kāi)發(fā)和引進(jìn)以16、52、58和53型為主的HPV四聯(lián)預(yù)防性疫苗,可降低59.47%的HPV人群感染;如果以降低宮頸癌變?yōu)橹饕康?應(yīng)考慮開(kāi)發(fā)和引進(jìn)以16、31、58和18型為主的HPV四聯(lián)預(yù)防性疫苗,可降低81.57%CINⅢ以上宮頸病變的發(fā)生。
