【摘要】：目的探討乙肝病毒攜帶對不同卵巢儲備功能女性進行體外受精-胚胎移植(IVF-ET)助孕胚胎發(fā)育及妊娠結局的影響。方法回顧性分析接受IVF-ET助孕的1 310例不孕癥患者資料,按照血清抗苗勒管激素(AMH)水平,分為三組:卵巢儲備低下組(A組)AMH2μg/L,正常卵巢儲備組(B組)AMH 2~7μg/L,卵巢高儲備組(C組)AMH7μg/L,每組按HBs Ag檢測結果分為女方乙肝病毒攜帶組和正常對照組,比較兩組的正常受精率、卵裂率、優(yōu)質(zhì)胚胎率、著床率、臨床妊娠率及流產(chǎn)率有無差異。結果 A組及C組中,乙肝病毒攜帶組與正常對照組的正常受精率、卵裂率、優(yōu)質(zhì)胚胎率、著床率及臨床妊娠率差異無統(tǒng)計學意義(P0.05),但A組中乙肝病毒攜帶組的無可移植胚胎率(14.6%)高于正常對照組(5.9%)(P0.05);B組中乙肝病毒攜帶組的正常受精率(69.8%)、卵裂率(96.97%)、優(yōu)質(zhì)胚胎率(21.8%)、著床率(31.3%)及臨床妊娠率(44.1%)均低于正常對照組(73.6%,98.6%,26.2%,41.3%,59.8%)(P0.05),差異有統(tǒng)計學意義。3組中乙肝病毒攜帶組的流產(chǎn)率均高于正常對照組(P0.05),差異有統(tǒng)計學意義。結論乙肝病毒攜帶可能干擾了卵巢儲備低下及正常卵巢儲備女性的卵子質(zhì)量及胚胎的發(fā)育,最終影響妊娠結局。
[Abstract]:Objective to investigate the effect of hepatitis B virus carrier on the development and outcome of in vitro fertilizing and embryo transfer (IVF-ET) assisted pregnancy in women with different ovarian reserve functions. Methods the data of 1 310 infertile patients receiving IVF-ET assisted pregnancy were analyzed retrospectively. according to the level of serum anti-Muller tube hormone (AMH), they were divided into three groups: low ovarian reserve group (group A), normal ovarian reserve group (group B) AMH2 ~ 7 渭 g / L, ovarian high reserve group (group C) AMH 7 渭 g / L. According to the results of HBs Ag test, each group was divided into female hepatitis B virus carrier group and normal control group. There were no differences in cleavage rate, high quality embryo rate, implantation rate, clinical pregnancy rate and abortion rate. Results there was no significant difference in normal fertilizing rate, cleavage rate, high quality embryo rate, implantation rate and clinical pregnancy rate between group A and group C, but the rate of untransplantable embryo in group A (14.6%) was higher than that in normal control group (5.9%) (P 0.05). The normal fertilizing rate (69.8%), cleavage rate (96.97%), high quality embryo rate (21.8%), implantation rate (31.3%) and clinical pregnancy rate (44.1%) in group B were lower than those in normal control group (73.6%, 98.6%, 41.3%, 59.3%, P 0.05). The abortion rate of hepatitis B virus carrier group was higher than that of normal control group (P 0.05), and the difference was statistically significant. Conclusion Hepatitis B virus carrier may interfere with egg quality and embryo development in women with low ovarian reserve and normal ovarian reserve, and finally affect pregnancy outcome.
